目的探讨半胱氨酸丰富蛋白Kazal基元(RECK)及基质金属蛋白酶-2(MMP-2)的表达与口腔鳞癌发生、发展及侵润、转移的关系。方法选择中国医科大学附属口腔医院病理科存档中2008年9月至2009年10月石蜡包埋的口腔鳞癌术后标本68例为实验组,对应的远端正常口腔黏膜组织68例为对照组。应用免疫组化SP法检测RECK及MMP-2蛋白表达,并在显微镜下观察阳性细胞所占百分比及着色深浅进行结果判定。结果癌组织中RECK蛋白的阳性表达率为40.4%,明显低于正常口腔黏膜组织83.8%(χ2=11.284,P<0.01)。RECK的表达与组织学分级、侵润深度、淋巴结转移相关(P均<0.05)。癌组织中MMP-2蛋白的阳性表达率为76.5%,明显高于正常口腔黏膜组织27.9%(P<0.01)。MMP-2蛋白阳性表达随着组织学分级、侵润深度级别的增加逐渐升高(P均<0.05);有淋巴结转移的MMP-2蛋白阳性表达率高于无淋巴结转移组(P<0.05)。RECK蛋白表达与MMP-2蛋白表达呈负相关关系(r=-0.5426)。结论 RECK和MMP-2在口腔鳞癌的侵润、转移过程中具有重要作用,RECK及MMP-2的联合检测可望成为口腔鳞癌早期诊断和判断预后的分子指标。
Objective To investigate the relationship between the expression of RECK and MMP-2 and the occurrence, development, invasion and metastasis of oral squamous cell carcinoma. Methods Seventy-eight specimens of paraffin-embedded oral squamous cell carcinoma (OSCC) from September 2008 to October 2009 in Archives of Pathology of Stomatology Hospital Affiliated to China Medical University were selected as experimental group and 68 corresponding normal oral mucosa tissues as control group . Immunohistochemical SP method was used to detect the expression of RECK and MMP-2, and to observe the percentage of positive cells under microscope and the color depth. Results The positive expression rate of RECK protein in cancer tissues was 40.4%, which was significantly lower than that in normal oral mucosa tissues (χ2 = 11.284, P <0.01). RECK expression was correlated with histological grade, depth of invasion and lymph node metastasis (all P <0.05). The positive expression rate of MMP-2 protein in cancer tissue was 76.5%, which was significantly higher than that in normal oral mucosa (27.9%, P <0.01). The positive expression rate of MMP-2 protein gradually increased with the histological grade and depth of invasion (all P <0.05). The positive rate of MMP-2 protein with lymph node metastasis was higher than that without lymph node metastasis (P <0.05) . RECK protein expression and MMP-2 protein expression was negatively correlated (r = -0.5426). Conclusions RECK and MMP-2 play an important role in the invasion and metastasis of oral squamous cell carcinoma. Combined detection of RECK and MMP-2 is expected to be a molecular marker for early diagnosis and prognosis of oral squamous cell carcinoma.
