1概述个案管理(casemanagement)的服务形式用于精神卫生领域最早是在20世纪60年代。当时精神卫生服务的主流是将住院机构大量关闭(非住院化运动),发展以社区为基础的服务模式。其目的是避免多种社区服务的相互脱节,提高社区眼务质量,以满足病人的多种需求。个案管理有5大功能,即:评估病人的需求;制定计划以满足上述需求;提供综合服务;监督并评估服务体系;随访并对病人进行评价
1 Overview The case management service model was first used in the mental health field in the 1960s. At the time, the mainstream of mental health services was to shut down a large number of inpatient institutions (non-institutionalized sports) and develop community-based service models. Its purpose is to avoid the disconnection of multiple community services and improve the quality of community eye care to meet the multiple needs of patients. Case management has five functions, namely: assessing patient needs; planning to meet the above needs; providing comprehensive services; monitoring and evaluating service systems; follow-up and evaluation of patients
