目的:通过研究肘管解剖基础,设计一种符合解剖、生物力学且操作简单、疗效满意的手术方案。方法:根据肘管解剖基础特点及致病因素,设计一种小切口,尺神经沟扩大成形术式。自2002年12月至2007年2月对23例肘管综合征患者,按该术式应用于临床观察疗效。本组按Machinnin和Dellon肘部尺神经卡压分度标准,中度6例、重度17例。结果:术后随访10个月至43个月,平均19.3个月。23例肘管综合征患者临床症状全部消失,功能恢复正常。结论:尺神经沟扩大成形术扩大了肘管的四壁,保留了正常的软组织床,解决了屈肘时尺神经的牵拉张力,改善了神经内的微循环,符合解剖和生物力学,是一种操作简单,创伤小的手术方法。
OBJECTIVE: To design an anatomical and biomechanical surgical solution with simple operation and satisfactory curative effect by studying the anatomy of elbow canal. Methods: According to the anatomic basis of elbow anatomy and pathogenic factors, a small incision and ulnar nerve enlargement was designed. From December 2002 to February 2007 in 23 patients with elbow syndrome, according to the technique used in clinical observation of efficacy. The group according to Machinnin and Dellon elbow ulnar nerve compression indexing standards, 6 cases were moderate and severe in 17 cases. Results: The patients were followed up for 10 months to 43 months with an average of 19.3 months. The clinical symptoms of 23 patients with elbow syndrome disappeared completely and the function returned to normal. CONCLUSION: Enlarged ulnar nerve enlargement enlarges the four walls of the elbow, preserves the normal soft tissue bed, solves the pulling tension of the ulnar nerve during flexion and improves the microcirculation within the nerve in line with anatomy and biomechanics. A simple, less invasive surgical methods.
