患者女,59岁,近1个月反复出现腹痛、腹胀,腹痛为阵发性,以剑突下及下腹部为主,伴嗳气、排便困难及肛门坠胀感。实验室检查:大便OB(+);FIB4.91 g/L。肠镜检查:回肠末段-回盲部-升结肠静脉显露。腹部X线立位片以及肝胆脾胰、双肾、输尿管、盆腔超声检查均未发现异常。胃肠造影:十二指肠螺旋形下降(图1)。CT表现:十二指肠水平段向肠系膜上动脉前方绕行,与肠系膜上静脉和部分肠系膜上动脉分支以肠系膜上动脉主干为轴顺时
Female patient, 59 years old, nearly 1 month recurrent abdominal pain, abdominal distension, abdominal pain as paroxysmal, mainly under the xiphoid and lower abdomen, with belching, defecation difficulty and anal bulge. Laboratory tests: stool OB (+); FIB 4.91 g / L. Colonoscopy: the final ileum - ileocecal - ascending colon vein revealed. Abdominal X-ray and liver and gallbladder spleen and pancreas, kidney, ureter, pelvic ultrasound examination were found no abnormalities. Gastrointestinalography: duodenal spiral decline (Figure 1).
