目的 探讨星形细胞瘤动态增强MRI与血管内皮生长因子 (VEGF)表达的相关性。资料与方法 对 33例星形细胞瘤术前采用TurboFLASH序列进行动态增强扫描 ,通过对时间 信号强度曲线分析 ,获得对比剂首过期曲线的最大斜率 (SSmax)和Tm1两参数。术后取病检分别进行VEGF和微血管密度 (MVD)免疫组织化学染色 ,计算每一标本的VEGF表达程度及MVD。将VEGF表达与SSmax、Tm1分别进行相关分析。结果 VEGF不同表达程度的星形细胞瘤之间的SSmax或Tm1差异有统计学意义 (P <0 .0 5 )。星形细胞瘤的SSmax与VEGF表达程度成正相关 ,rs值为 0 .6 8;Tm1与VEGF表达程度成负相关 ,rs 值为 - 0 .6 97,P值均 <0 .0 0 1。VEGF表达程度与MVD存在相关性。结论 星形细胞瘤的VEGF表达程度与SSmax、Tm1之间存在显著性的相关 ,其病理生理学基础是VEGF促进星形细胞瘤血管生成。对星形细胞瘤动态增强MRI曲线分析有助于预测星形细胞瘤的VEGF表达
Objective To investigate the correlation between dynamic enhanced astrocytoma (MRI) and vascular endothelial growth factor (VEGF) expression. Materials and Methods Thirty-three cases of astrocytoma were dynamically scanned by TurboFLASH before and after treatment. The maximal slope (SSmax) and Tm1 of the first transition curve were obtained by analyzing the time signal intensity curve. Pathological examination after surgery were performed VEGF and microvessel density (MVD) immunohistochemical staining VEGF levels were calculated for each specimen and MVD. The correlation between VEGF expression and SSmax and Tm1 were analyzed respectively. Results There were significant differences in the values of SSmax or Tm1 between astrocytomas with different expression levels of VEGF (P <0.05). The positive expression of SSmax in astrocytomas was positively correlated with the expression of VEGF, with a rs of 0.68. Tm1 was negatively correlated with the expression of VEGF, rs was -0.667, P <0.01. There is a correlation between the expression of VEGF and MVD. Conclusion There is a significant correlation between the expression of VEGF and the levels of SSmax and Tm1 in astrocytoma. The pathophysiology of VEGF is that VEGF promotes astrocytoma angiogenesis. Dynamic enhancement of astrocytoma MRI curve analysis can help predict the expression of VEGF in astrocytomas
