目的 探讨参附注射液对脊髓缺血性损伤保护作用的剂量效应关系。方法 2 4只雄性新西兰大白兔 ,随机分为四组 :A组 (n =6 ) ,单纯缺血再灌注组 ,B组 (n =6 )、C组 (n =6 )和D组 (n=6 )分别于缺血前 30min内持续恒速静脉输入参附注射液 5ml·kg-1、10ml·kg-1、2 0ml·kg-1;采用肾下主动脉阻断法造成脊髓缺血 (2 0min) ;术后观察神经功能变化并记录再灌注 1h、4h、8h、12h、2 4h和 48h神经功能评分 ,再灌注 48h处死动物后取脊髓 (L5~ 7)标本行病理学观察。结果 术后神经功能评分除再灌注 1hD组与A组之间无显著性差异 ,其余各时间点B组、C组和D组均明显高于A组 (P <0 0 5 ) ,B组、C组和D组间则无显著性差异 (P >0 0 5 ) ,且 48h时各组神经功能评分分别为A组 0 5±0 8、B组 3 2± 0 9(P <0 0 0 1)、C组 3 0± 1 0 ,(P <0 0 0 1)和D组 2 7± 0 8(P <0 0 5 ) ;再灌注 48h脊髓前角正常运动神经元计数B组 (78± 2 5 ,P <0 0 0 1)、C组 (41± 2 3,P <0 0 5 )和D组 (42± 2 7,P <0 0 5 )均高于A(8± 7)组 ,B组与C组、D组间亦有显著性差异 (P <0 0 5 )。结论 参附注射液对脊髓缺血性损伤有保护作用 ,但无明显剂量效应关系。
Objective To investigate the dose-response relationship of Shenfu injection on protection of ischemic spinal cord injury. Methods 24 male New Zealand white rabbits were randomly divided into four groups: group A (n = 6), simple ischemia-reperfusion group, group B (n = 6), group C (n = 6) and group D (n =6)Continuously infusion of Shenfu injection (5ml·kg-1, 10ml·kg-1, and 20ml·kg-1) in 30 min before ischemia, and using ischemic infrarenal occlusion to cause spinal cord ischemia. (2 0min) Postoperative neurological changes were observed and neurological scores at 1h, 4h, 8h, 12h, 24h and 48h after reperfusion were recorded. Animals were sacrificed 48 h after reperfusion and spinal cord (L5-7) specimens were taken for pathological observation. Results There was no significant difference in postoperative neurological function scores, except for 1 hour of reperfusion. There was no significant difference between group D and group A. At other time points, group B, group C, and group D were significantly higher than group A (P < 0.05). There was no significant difference between group C and group D (P > 0 05). At 48 hours, neurological function scores in each group were 0 5 ± 0 8 in group A and 3 2 ± 0 9 in group B (P <0 0 0 1) In group C, 30 ± 10, (P <0 0 01) and in group D 2 7 ± 0 8 (P <0 05); In 48h of reperfusion, normal motor neurons in the anterior horn of spinal cord were counted in group B (78 ± 25, P <0 0 0 1), C (41 ± 2 3, P <0 05) and D (42 ± 2 7, P <0 05) all higher than A (8 ± 7) There was also a significant difference between group B and group C and D (P < 0.05). Conclusion Shenfu injection has a protective effect on ischemic spinal cord injury, but there is no obvious dose-effect relationship.
