目的了解男性性服务者HIV、梅毒感染率及其影响因素,可以为开展有针对性的干预工作提供建议。方法通过滚雪球的方式招募调查对象,由经培训后的调查员进行面对面问卷调查,采集血样检测HIV和梅毒,采用逐步后退的Logistic回归分析鉴别影响HIV、梅毒感染及保护性性行为的因素。结果共计调查男性性服务者190人,年龄(24.2±6.0)岁,最近6个月平均男性性伴数为8个,最近一次与男性发生肛交时安全套使用率为73.7%,最近6个月与男性发生保护性肛交的比例为44.7%。HIV感染率为11.6%(22/190),梅毒感染率为11.1%(21/190)。梅毒感染是影响男性性服务者人群HIV感染的独立影响因素(OR=3.8,95%CI:1.3~11.2)。高文化程度(OR=1.5,95%CI:0.5~4.5)和HIV感染(OR=4.7,95%CI:1.5~15.3)是影响梅毒感染的独立影响因素。未婚(OR=4.9,95%CI:1.5~15.9)与首次性行为性伴是女性(女性vs男性:OR=2.8,95%CI:1.5~5.1)是影响保护性性行为的独立影响因素。结论男性性服务者HIV和梅毒感染率高,性伴多,与男性发生肛交时安全套使用率低,将促进艾滋病在MSM人群中的快速传播,并作为桥梁人群促进艾滋病由MSM人群向普通人群传播。
Objective To understand the prevalence of HIV and syphilis in male sex workers and its influencing factors, and to provide suggestions for carrying out targeted interventions. METHODS: The subjects were recruited by means of snowballs. The trained investigators conducted face-to-face questionnaires to collect blood samples for HIV and syphilis. Logistic regression analysis was used to identify the factors influencing HIV, syphilis infection and protective behaviors. Results A total of 190 male sexual service workers were investigated, with an average age of (24.2 ± 6.0) years. The average number of male partners was 8 in the last 6 months. The most recent condom use rate was 73.7% for anal intercourse with men. In the recent 6 months, The percentage of protective anal sex in men was 44.7%. The HIV infection rate was 11.6% (22/190) and syphilis infection rate was 11.1% (21/190). Syphilis infection is an independent influencing factor of HIV infection in male sex workers (OR = 3.8, 95% CI: 1.3-11.2). High education (OR = 1.5, 95% CI: 0.5-4.5) and HIV infection (OR = 4.7,95% CI: 1.5-15.3) were the independent influencing factors of syphilis infection. Women who were unmarried (OR = 4.9, 95% CI 1.5 to 15.9) and first-time sex partners were female (vs vs male: OR = 2.8, 95% CI: 1.5 to 5.1) were independent predictors of protective sexual behavior. Conclusions Male sex workers have high rates of HIV and syphilis infection, high number of sexual partners and low condom use rate in anal intercourse with men, which will promote the rapid transmission of AIDS among MSM population and serve as a bridge to promote the spread of AIDS among MSM population to the general population .
