目的:研究便携式加速度记录定量化评价帕金森病(PD)患者运动障碍程度的特异性。方法:61例PD患者根据国际治疗指南接受常规治疗,于治疗前与治疗后(30~60d后)将便携式加速度记录仪佩戴于患者利手侧手腕连续记录6d。同时利用帕金森病统一评分量表(UPDRS)评价治疗前后PD患者的临床评分;同时解析加速度记录仪记录的身体活动数据,评价PD患者治疗前后的幂型自相关指数(PLE)的变化,分析UPDRS评分与PLE的相关性。结果:与治疗前比较,治疗后PD患者的UPDRS评分以及PLE值均显著改善(P<0.05),PLE改善率与UPDRS评分以及运动障碍部分(PartⅢ)评分的改善率呈直线相关(分别r=0.794和0.823,P<0.01);结论:利用PLE解析加速度记录仪所记录的身体运动可以用于客观定量化评价PD运动障碍的程度,有较高的特异性,可作为PD患者门诊疗效评价的工具。
PURPOSE: To study the specificity of portable acceleration recording for the quantitative assessment of the degree of dyskinesia in Parkinson’s disease (PD). Methods: Sixty-one patients with PD underwent routine treatment according to the international guidelines for treatment. Before and after the treatment (after 30-60 days), the portable accelerograph was worn on the wrist of the patient’s hand for 6 days. At the same time, UPDRS was used to evaluate the clinical scores of PD patients before and after treatment. At the same time, the physical activity data recorded by accelerometer were analyzed to evaluate the change of power-type autocorrelation index (PLE) before and after PD treatment. Correlation of UPDRS score with PLE. Results: Compared with those before treatment, the UPDRS score and PLE of PD patients were significantly improved (P <0.05), and the improvement rate of PLE was linearly correlated with UPDRS score and improvement rate of dyskinesia (Part Ⅲ) score (r = 0.794 and 0.823, P <0.01). CONCLUSION: Body movement recorded by PLE accelerometer can be used to objectively and quantitatively evaluate the degree of PD dyskinesia. It has high specificity and can be used as an outpatient therapeutic evaluation for PD patients tool.
