目的通过检查我院门诊中药处方质量及合理用药,以减少医疗差错发生,使处方的书写质量得到提高,处方配伍酌情合理。方法随机抽取2005年共9900张处方进行统计分析。结果不合格处方占总数的13.20%,其中处方书写不合格占2.34%,用药剂量不当占1.57%,脚注不明占2.86%,配伍不合理1.53%,用法不明占0.3%,药名不规范占4.60%,药味在22味以上者占6.86%。结论根据处方在书写、用药及配伍方面存在的诸多问题,建议制定医院用药手册和合理用药的评判标准,通过定期药讯和用药知识讲座,提高药剂人员素质,增强医师用药知识。
Objective To reduce the medical errors by checking the prescription quality and reasonable medication of outpatient Chinese medicine in our hospital so that the writing quality of the prescription can be improved and the prescription compatibility is reasonable. Methods A total of 9900 prescriptions were randomly selected for statistical analysis in 2005. The results of substandard prescriptions accounted for 13.20% of the total, of which 2.34% failed to write prescriptions, improper dosage of 1.57%, unspecified accounts for 2.86%, compatibility unreasonable 1.53%, accounting for 0.3% unknown, non-standard medicine accounted for 4.60 %, Medicine taste more than 22 taste 6.86%. Conclusion According to the many problems in prescription, medication and compatibility of prescriptions, it is suggested to set the hospital medication manual and the evaluation criteria of rational drug use, and to improve the quality of pharmacy staff and enhance the knowledge of physicians through regular information and medication knowledge lectures.
