目的探讨川崎病患儿血清抗内皮细胞抗体和抗中性粒细胞胞浆抗体检测的临床意义。方法选取2016年5月至2017年5月本院收治的川崎病患儿25例,作为研究组,应用酶联免疫吸附试验定量检测60例川崎病患儿急性期、缓解期血清抗内皮细胞抗体和抗中性粒细胞胞浆抗体水平,并行心脏超声心动图了解冠状动脉损害(CAL)发生情况治疗。随机选取同期因呼吸道感染发热患儿25例作为对照组应用酶联免疫吸附试验定量检测血清抗内皮细胞抗体和抗中性粒细胞胞浆抗体水平。结果在研究组中急性期川崎病患儿血清抗内皮细胞抗体和抗中性粒细胞胞浆抗体水平均显著高于对照组(P<0.05)。在研究组中,与急性期相比,缓解期血清血清抗内皮细胞抗体和抗中性粒细胞胞浆抗体水平下降,但仍高于对照组(P<0.05)。伴有冠状动脉损害的川崎病患儿急性期血清抗内皮细胞抗体水平明显高于无冠状动脉损害的患儿(P<0.05)。结论抗内皮细胞与抗中性粒细胞胞浆抗体可能与川崎病血管炎及其冠状动脉损伤的发生有关。急性期血清中抗内皮细胞抗体、抗中性粒细胞胞浆抗体对早期难以确诊的川崎病有参考价值,抗内皮细胞抗体含量的增高对冠状动脉损害有一定的预警价值。
Objective To investigate the clinical significance of serum anti-endothelial cell antibody and anti-neutrophil cytoplasmic antibody in children with Kawasaki disease. Methods Twenty-five children with Kawasaki disease admitted to our hospital from May 2016 to May 2017 were enrolled in this study. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of serum anti-endothelial cell antibodies And anti-neutrophil cytoplasmic antibody levels, parallel echocardiography to understand the occurrence of coronary artery lesions (CAL) treatment. Twenty-five children with fever due to respiratory infection were randomly selected as the control group. Serum levels of anti-endothelial cell antibodies and anti-neutrophil cytoplasmic antibodies were measured by enzyme-linked immunosorbent assay (ELISA). Results The levels of serum anti-endothelial cell antibodies and anti-neutrophil cytoplasmic antibodies in children with acute Kawasaki disease in the study group were significantly higher than those in the control group (P <0.05). In the study group, levels of serum anti-endothelial cell antibodies and anti-neutrophil cytoplasmic antibodies in remission decreased compared with those in the acute phase, but still higher than those in the control group (P <0.05). The level of serum anti-endothelial antibody in Kawasaki disease patients with coronary artery lesions was significantly higher than that in children without coronary artery lesions (P <0.05). Conclusion Anti-endothelial cells and anti-neutrophil cytoplasmic antibodies may be related to the occurrence of Kawasaki disease vasculitis and coronary artery injury. Acute serum anti-endothelial cell antibodies, anti-neutrophil cytoplasmic antibodies early diagnosis of Kawasaki disease has a reference value, anti-endothelial cell antibody levels increased coronary damage has some warning value.
