乙胺碘呋酮对松弛冠状血管平滑肌具有高度的特异性,因此从1965年起已用于抗心绞痛。该药治疗心绞痛的药理作用为:(1)心率明显减慢;(2)短暂和中度的收缩压降低;(3)心肌耗氧量减少;(4)冠状血管阻力明显降低;(5)有部份非竞争性抑制儿茶酚胺的作用,因而能拮抗α-受体和β-受体。乙胺碘呋酮对血管痉挛性心绞痛的效果尤为显著。本文报道3例患者应用乙胺碘呋酮的疗效。方法:例1为57岁男性,3月来每于深夜出现剧烈的胸骨后疼痛。体检和常规心电图均正常,活动平板运动试验阴性。冠状动脉造影示,左冠状动脉主干、右冠状动脉近侧1/3和左冠状动脉回旋支狭窄约50%。麦角新碱试验在0.2毫克时即为阳
Amiodarone is highly specific for relaxing coronary artery smooth muscle and has been used in antianginal pain since 1965. The pharmacological effects of the drug in the treatment of angina pectoris are as follows: (1) heart rate slowed down significantly; (2) transient and moderate systolic blood pressure depression; (3) myocardial oxygen consumption decreased; (4) coronary vascular resistance decreased significantly; (5) Some non-competitive inhibition of catecholamines, which can antagonize the α-receptor and β-receptor. The effect of amiodarone on vasospastic angina is particularly pronounced. This article reports the efficacy of amiodarone in 3 patients. Methods: Example 1 was a 57-year-old man who developed severe post-sternal pain in late March every three months. Physical examination and routine ECG were normal, active treadmill test negative. Coronary angiography showed that the left main coronary artery, proximal 1/3 of the right coronary artery and left coronary artery circumflex artery stenosis were about 50%.
