目的总结医院门诊药房处方调剂差错原因,提出相应方法措施,降低门诊药房处方调剂差错,提高处方调剂工作质量。方法回顾性总结分析医院门诊药房2011-2014年查出的调剂差错处方共136张,对所有处方的调剂差错种类及原因进行统计分析。结果 2011-2014年发生的调剂出错处方136张,差错率逐年递减,门诊处方调剂差错原因有药师、医师、收费、患者等5个方面原因。结论针对处方调剂差错发生的常见原因采取一系列的有效防范措施,加强管理,提高医师及药师的责任心,减少差错发生几率,保障患者用药安全性。
OBJECTIVE To summarize the reasons for the errors in prescription dispensation of outpatient pharmacies in hospitals and to put forward the corresponding methods and measures to reduce the errors of prescription dispensation in outpatient pharmacies and improve the quality of prescription adjustment. Methods Retrospective analysis of the hospital outpatient pharmacies in 2011-2014 detected a total of 136 prescriptions errors, prescriptions for all types of errors and the reasons for the statistical analysis. Results There were 136 misdirected prescriptions in 2011-2014, the error rate decreased year by year. There were five reasons for errors in outpatient prescriptions adjustment, such as pharmacists, doctors, fees and patients. Conclusion A series of effective precautionary measures are taken to prevent common errors caused by prescription adjustment. Management is strengthened to increase the responsibility of physicians and pharmacists, reduce the risk of errors and ensure the safety of patients.
