本文测定了27例COPD患者葡萄糖耐量试验、胰岛素释放试验和C肽释放试验,并与20例正常人作对照研究,旨在探讨胰岛β细胞的功能。结果发现:①COPD组服糖后30min、60min和120min血糖值显著高于对照组(P<0.01),且血糖高峰后移,出现糖耐量减退者占48.15%;②COPD组血清胰岛素分泌高峰延迟,30min值明显低于对照组(P<0.05)而180min值却非常显著高于对照组(P<0.01),但两组胰岛素面积相差并不明显(P<0.05);③两组C肽各时相值差别无统计学意义(P>0.05),COPD组C肽高峰延迟,但两组C肽面积相差甚小(P>0.05)。本文认为COPD糖耐量减低主要是由于胰岛β细胞功能减退而致胰岛素分泌延迟以及存在胰岛素抗力的缘故。
In this study, we measured glucose tolerance test, insulin release test and C-peptide release test in 27 patients with COPD and compared them with 20 normal subjects to explore the function of islet β cells. The results showed that: (1) The serum glucose levels of 30d, 60min and 120min in COOPD group were significantly higher than those in control group (P <0.01), and the peak of blood glucose was shifted backward, accounting for 48.15% of those with impaired glucose tolerance; (P <0.05), while the value of 180min was significantly higher than that of the control group (P <0.01), but the difference of insulin area between the two groups was not significant (P <0.05) There was no significant difference between the two groups (P> 0.05). The peak of C peptide in COPD group was delayed, but the area of C peptide in the two groups had little difference (P> 0.05). This article suggests that COPD impaired glucose tolerance is mainly due to delayed beta-cell dysfunction caused by insulin secretion and insulin resistance due to the reason.
