本文报道应用小量氯丙嗪控制喉显微手术中心血管副反应的临床效果。选择ASAI~Ⅱ级喉显微手术病人34例,随机分为:Ⅰ组为对照组;Ⅱ组静注氯丙嗪5mg为实验组。麻醉后行高频喷射通气,静注氯丙嗪5mg,记录诱导前、插管后、静注氯丙嗪后1、3、5、10min时SBP、DBP、HR、SpO2数值。结果:实验组在高频喷射通气后,置显微喉镜前,静脉注射小量氯丙嗪5mg能有效地缓解和控制喉显微手术中血压剧烈变化。与对照组比较1min时SBP下降16.29%,DBP下降15.10%,HR无显著变化。用药后各时点血压与对照组比较统计学差异显著,以3~5min时为著,说明静脉注射小量氯丙嗪对控制喉显微手术中血压剧增效果确切,而对心率和术后苏醒无明显影响。
This article reports the clinical application of a small amount of chlorpromazine to control cardiovascular adverse reactions in laryngeal microsurgery. Thirty-four patients with ASAI-Ⅱ throat microsurgery were selected and randomly divided into group Ⅰ as control group, group Ⅱ as chlorpromazine 5 mg as experimental group. After anesthesia, high-frequency jet ventilation was performed. Chlorpromazine 5 mg was injected intravenously. The values of SBP, DBP, HR and SpO2 were recorded before and after induction of chlorpromazine 1, 3, 5 and 10 min after induction. Results: In the experimental group, a small amount of chlorpromazine 5 mg intravenously before high-frequency jet ventilation and micro-laryngoscope treatment can effectively relieve and control the dramatic changes of blood pressure in laryngeal microsurgery. Compared with the control group, the SBP decreased 16.29% and the DBP decreased 15.10% at 1 min, with no significant change in HR. Blood pressure at each time point after treatment was statistically significant difference compared with the control group, to 3 ~ 5min when, indicating that intravenous injection of a small amount of chlorpromazine for the control of laryngeal microsurgical surge in the exact effect of blood pressure, and heart rate and postoperative Wake up no significant effect.
