目的结合临床实践经验,探讨双侧臂丛阻滞加氯胺酮基础麻醉在小儿双手多指畸形手术中的应用效果。方法以我院小儿双手多指畸形手术患者45例为研究对象,将患儿随机均分为1组和2组,手术中分别采用不同的麻醉方法,比较两组患儿麻醉后的MAP、HR、SpO2、苏醒时间以及不良反应情况,探讨两种麻醉方法的有效性。结果 1组麻醉后MAP、HR较2组高、SpO2较2组低,差异有统计学意义;1组苏醒所需时间长于2组,差异有统计学意义;1组有较多的不良反应出现,而2组则较少,差异有统计学意义。结论小儿双手多指畸形手术用双侧臂丛阻滞(辅助氯胺酮基础麻醉)优于单纯氯胺酮麻醉。其麻醉效果完善,具有并发症少、安全性高的优点,是进行小儿上肢双手多指畸形手术中一种不错的麻醉方法,值得临床推广。
Objective To evaluate the clinical effects of bilateral brachial plexus block plus ketamine on anesthesia in children with both hands and multiple finger deformities. Methods 45 cases of pediatric multi-finger deformity surgery in our hospital were studied. The children were randomly divided into 1 group and 2 groups. Different anesthesia methods were used during the operation. MAP, HR , SpO2, recovery time and adverse reactions, to explore the effectiveness of the two anesthesia methods. Results The MAP and HR of group 1 were higher than those of group 2 after anesthesia, while the value of SpO2 was lower than that of group 2, the difference was statistically significant. The time required for recovery in group 1 was longer than that of group 2, with statistically significant difference. More adverse reactions occurred in group 1 , While the two groups are less, the difference was statistically significant. Conclusions The bilateral brachial plexus block (assisted with ketamine base anesthesia) is superior to simple ketamine anesthesia in pediatric multi-finger deformity surgery. Its anesthetic effect is perfect, with fewer complications and high safety. It is a good anesthetic method in pediatric multi-finger deformity surgery and is worthy of clinical promotion.
