目的:探讨发生围术期并发症是否对食管癌远期预后产生影响。方法:通过回顾性分析2012年1月-2013年12月在我科进行食管癌根治手术并成功随访的患者,观察远期复发的危险因素及围术期并发症对远期预后的影响。结果:177例食管癌患者纳入本研究,平均随访28个月,共有88例(49.7%)患者出现复发。肿瘤浸润深度、淋巴结转移、神经脉管侵犯、吻合口残端阳性及围术期并发症为术后肿瘤复发的危险因素。其中术后并发症≥ClavienⅡ级是食管癌远期复发的独立预测因子。结论:围术期并发症会影响患者的远期预后,缩短无复发生存。
Objective: To investigate whether perioperative complications affect the long-term prognosis of esophageal cancer. Methods: We retrospectively analyzed the risk factors for long-term recurrence and the effect of perioperative complications on long-term prognosis in patients undergoing radical resection of esophageal cancer from January 2012 to December 2013 in our department. RESULTS: A total of 177 patients with esophageal cancer were enrolled in this study, with an average follow-up of 28 months. A total of 88 patients (49.7%) developed recurrence. The depth of tumor invasion, lymph node metastasis, neurovascular invasion, anastomotic stump positivity and perioperative complications are the risk factors for postoperative tumor recurrence. Which postoperative complications ≥ Clavien Ⅱ level is an independent prognostic factor for long-term esophageal cancer recurrence. Conclusion: Perioperative complications will affect the long-term prognosis of patients and shorten the recurrence-free survival.
