目的探索哮喘和抑郁二者共病的可能机制。方法使用自制量表、哮喘控制测试表(ACT)、哮喘生存质量问卷(AQLQ)、汉密尔顿抑郁量表(HRSD)对受试者(分为哮喘组、哮喘伴抑郁组、正常对照组)的一般情况及哮喘严重程度、哮喘控制情况、哮喘生活质量、心理卫生情况进行评估。对哮喘及哮喘伴抑郁组受试者测定肺功能及血中嗜酸性粒细胞及IgE水平。所有受试者的血样提取分选CD4+T细胞,行实时荧光定量聚合酶链式反应(real time-PCR)检测SLC6A4、HTR7、HTR1A、HTR2A基因的表达水平。结果哮喘组及哮喘伴抑郁组SLC6A4基因的相对表达量低于正常对照组(P=0.000);哮喘组HTR2A基因的表达量低于正常对照组(P=0.021),哮喘组HTR2A基因表达有低于哮喘伴抑郁组的趋势(P=0.077);哮喘伴抑郁组较哮喘组有更低的哮喘生活质量评分(P=0.004)。结论支气管哮喘及抑郁可能存在基因水平的联系,SLC6A4基因的表达水平下降可能是二者共病的机制之一。
Objective To explore the possible mechanism of comorbidity between asthma and depression. Methods The subjects were divided into asthma group, asthma with depression group and normal control group using self-made scale, ACT, AQLQ, and HRSD. Condition and the severity of asthma, asthma control, quality of life of asthma, mental health assessment. Subjects with asthma and asthma with depression measured lung function and blood levels of eosinophils and IgE. CD4 + T cells were extracted from blood samples of all subjects, and the expression of SLC6A4, HTR7, HTR1A and HTR2A genes was detected by real time-PCR. Results The relative expression of SLC6A4 gene in asthma group and asthma with depression group was lower than that in normal control group (P = 0.000). The expression of HTR2A gene in asthma group was lower than that in normal control group (P = 0.021) (P = 0.077). Asthma with depression had a lower asthma quality of life score than asthma group (P = 0.004). Conclusions There may be genetic correlation between bronchial asthma and depression. The decreased expression of SLC6A4 gene may be one of the mechanisms of co-morbidities.
