目的探讨颈动脉交感神经网切除术对脑性瘫痪患儿吞咽障碍及流涎的改善作用。方法脑性瘫痪患儿42例,随机分为手术组及对照组各21例。对照组仅行康复治疗,手术组行颈动脉交感神经网切除术+康复治疗,分别采用洼田饮水试验(洼田评分)及Teacher流涎分级法(TDS)对手术组术前、术后2周、术后6个月及同期对照组的吞咽功能、流涎情况进行评价。结果手术组术后2周及6个月时洼田评分、TDS低于治疗前及对照组(P均<0.05)。两组治疗后6个月与治疗后2周各指标比较,P均﹥0.05。结论颈动脉交感神经网切除术可改善脑性瘫痪患儿吞咽功能,减少其流涎。
Objective To investigate the effect of carotid artery sympathectomy on dysphagia and salivation in children with cerebral palsy. Methods 42 cases of children with cerebral palsy were randomly divided into operation group and control group of 21 cases. The control group was treated only with rehabilitation. The operation group was treated with carotid sympathectomy and rehabilitation. The Kudzu water test (Kudan Score) and Teacher Salivation Grading (TDS) , 6 months after surgery and the control group swallowing function, salivation assessment. Results The Kawada score and TDS in the surgery group at 2 weeks and 6 months after operation were lower than those before treatment and in the control group (all P <0.05). The two groups after 6 months of treatment and 2 weeks after treatment each index, P> 0.05. Conclusion Carotid artery sympathectomy can improve swallowing function and reduce salivation in children with cerebral palsy.
