目的通过检测固定矫治初期患者龈沟液(gingival crevicular fluid,GCF)中Ⅰ型胶原交联羧基末端肽(type Ⅰ collage cross-linked-telopeptide,CTX)水平,从微观角度研究正畸矫治过程中牙齿和牙槽骨的改变。方法选择固定矫治患者11例,拔除上颌双侧第一前磨牙。试验采用自身对照设计,每位受试者安装片段弓,将上颌尖牙随机分为试验侧和对照侧。于安装矫治器前1周及加力后1、2、3、4周时收集GCF。实验室检测GCF中CTX的水平。结果试验侧CTX的浓度与质量在加力1周后达高峰侧之后逐渐降低,1、2周时与治疗前差异有统计学意义(P<0.05,P<0.01);试验侧与对照侧CTX浓度与质量的比较在加力1周时差异有统计学意义(P<0.05);GCF的质量,试验侧加力2周时与治疗前之间差异有统计学意义(P<0.05);对照侧治疗前后各时间点之间差异无统计学意义(P>0.05)。结论正畸矫治(加力过程中)可以导致GCF中CTX浓度和含量的显著性变化,CTX水平在加力后1周达到高峰,提示为临床骨吸收的显著时期。
Objective To detect the level of type Ⅰ collage cross-linked-telopeptide (CTX) in gingival crevicular fluid (GCF) in patients undergoing initial orthodontic treatment. To study the effects of orthodontic treatment on teeth And alveolar bone changes. Methods Eleven patients with fixed orthodontics were selected and the first premolar maxillary bilateral was removed. The trial was self-controlled design, each subject was installed fragment bow, the maxillary canines were randomly divided into test side and control side. GCF was collected 1 week prior to appliance placement and 1, 2, 3, 4 weeks after application of force. Laboratory testing of CTX levels in GCF. Results The concentration and mass of CTX on the experimental side gradually decreased after 1 week and reached the peak level at 1 week, and there was a significant difference between before and after 1,2 weeks (P <0.05, P <0.01) (P <0.05). The quality of GCF was significantly different from that before treatment (P <0.05) after two weeks of applying force on the test side (P <0.05) There was no significant difference between before and after time points (P> 0.05). Conclusion Orthodontic treatment (during the application of force) can lead to a significant change of CTX concentration and content in GCF. The level of CTX peaked at 1 week after application of force, suggesting a significant period of clinical bone resorption.
