目的探讨脐动脉舒张末期血流缺失(AEDV)对围产儿预后的影响。方法应用脉冲多普勒超声监测胎儿脐血流,对24例出现AEDV(AEDV组)的母儿的临床资料进行分析,并与未发生AEDV的987例(对照组)进行比较。结果AEDV组胎儿生长受限、子痫前期、羊水过少、双胎输血综合征、胎儿畸形、妊娠期糖尿病发生率以及围产儿死亡率与对照组比较差异有统计学意义(P<0.05)。结论胎儿脐血流监测中,出现AEDV改变,是母体-胎盘、胎儿-胎盘循环障碍的表现,提示围产儿结局不良。
Objective To investigate the effect of umbilical artery end-diastolic blood flow (AEDV) on the prognosis of perinatal infants. Methods Umbilical cord blood flow was monitored by pulsed Doppler sonography. The clinical data of 24 pregnant women with AEDV (AEDV group) were analyzed and compared with 987 non-AEDV patients (control group). Results The fetal growth restriction, preeclampsia, oligohydramnios, twin transfusion syndrome, fetal malformations, gestational diabetes mellitus and perinatal mortality in the AEDV group were significantly different from those in the control group (P <0.05). Conclusions AEDV changes in fetal umbilical cord blood flow monitoring are the manifestation of maternal-placenta, fetal-placental circulatory disturbance, suggesting poor perinatal outcome.
