急性有机磷农药中毒是临床常见危重疾病。有机磷农药中毒的机制是神经系统胆碱酯酶活性受抑制,胆碱酯酶磷脂化,无法水解乙酰胆碱,导致乙酰胆碱蓄积,从而引起一系列中毒症状。但目前对毒物的清除(包括胃肠道及血液内),对抗胆碱酯酶蓄积、恢复胆碱酯酶活性的药物使用,机械通气等关键问题认识不充分,基层医院死亡率仍高。现就上述问题结合本科救治经验进行总结,以期对提高临床抢救成功率有所帮助。
Acute organophosphorus pesticide poisoning is a common clinical critical disease. Organophosphorus pesticide poisoning mechanism is the inhibition of the nervous system cholinesterase activity, phospholipid of cholinesterase, can not hydrolyze acetylcholine, resulting in the accumulation of acetylcholine, which caused a series of symptoms of poisoning. However, the current clearance of poison (including the gastrointestinal tract and blood), against the accumulation of cholinesterase, the use of drugs to restore cholinesterase activity, inadequate understanding of the key issues such as mechanical ventilation, primary hospital mortality is still high. The above questions are summarized with the experience of undergraduate treatment, with a view to improve the success rate of clinical rescue helpful.
