目的:探讨睫状体黑色素细胞瘤的超声诊断特征,并与睫状体黑色素瘤进行鉴别。方法:回顾性分析2010年5月—2015年9月北京同仁医院眼科收治的睫状体黑色素细胞瘤患者22例(22只眼),同时选取同期就诊的睫状体黑色素瘤患者23例(23只眼)作为鉴别,所有患者均经病理诊断确诊。分析所有患者的超声生物显微镜(ultrasound biomicroscopy,UBM)、彩色多普勒血流成像及超声造影表现。结果:UBM检查显示21例睫状体黑色素细胞瘤中,呈高回声14例、中等回声4例、低回声3例,17例侵犯虹膜及房角,1例侵犯巩膜;15例睫状体黑色素瘤中,呈中等回声5例、低回声10例。彩色多普勒血流成像显示,22例睫状体黑色素细胞瘤中,形态呈梭形1例、半球形21例,15例病变内可见血流信号;23例睫状体黑色素瘤中,形态呈梭形1例、半球形15例、蕈状7例,22例病变内可见血流信号。超声造影检查显示,睫状体黑色素细胞瘤峰值强度低于睫状体黑色素瘤,平均渡越时间长于睫状体黑色素瘤,差异有统计学意义。结论:睫状体黑色素细胞瘤的超声检查有一定的特点,可为临床提供可靠的诊断和鉴别诊断依据。
Objective: To investigate the ultrasonic diagnostic features of ciliary body melanocytoma and differentiate it from ciliary body melanoma. Methods: Twenty-two patients (22 eyes) with ciliary body melanocytoma admitted to Beijing Tongren Hospital from May 2010 to September 2015 were retrospectively analyzed. Twenty-three patients (23 Eyes) as a differential, all patients diagnosed by pathological diagnosis. All patients underwent ultrasound biomicroscopy (UBM), color Doppler flow imaging and contrast-enhanced ultrasound findings. Results: UBM examination showed that in 21 cases of ciliary body melanocytoma, there were 14 cases of high echo, 4 cases of moderate echo, 3 cases of hypoechoic, 17 cases of iris and anterior chamber angle and 1 case of sclera. 15 cases of ciliary body melanin Tumors, showed moderate echo in 5 cases, 10 cases of hypoechoic. Color Doppler flow imaging showed that in 22 cases of ciliary body melanocytoma, the morphology was spindle-shaped in 1 case, hemispherical in 21 cases, 15 cases of pathological changes were seen in blood flow signals; 23 cases of ciliary body melanoma, morphology Fusiform 1 case, hemispherical 15 cases, 7 cases of mushroom, 22 cases of blood flow signal can be seen. Contrast-enhanced ultrasound showed that the peak intensity of ciliary body melanocytoma was lower than that of ciliary body melanoma, and the average transit time was longer than that of ciliary body melanoma. The difference was statistically significant. Conclusion: Ultrasound examination of ciliary body melanocytoma has certain characteristics, which can provide reliable diagnosis and differential diagnosis basis for clinical diagnosis.
