目的 :了解蒙自市抗病毒治疗门诊HIV、HCV双感染情况,及双感染者肝损伤程度。方法 :横断面调查方法,方便抽样抽取2014年11月期间门诊接受治疗的HIV感染者/AIDS病人作为调查对象,问卷调查的方式收集人口学,丙肝感染情况,谷丙转苷酶检测值,谷草转氨酶检测值,血小板检测值等信息。应用Epidata建立数据库,应用FIB4及APRI进行肝纤维化评估,SPSS17.0软件进行统计学分析。结果 :共调查121人,38人为丙肝阳性感染者,其中21名丙肝者来源于吸毒人群,按照APRI评分法,28人已经肝纤维化病变,2人为F2级以上肝纤维化,5人怀疑肝硬化。按照FIB-4评分法,6人肝纤维化为F3-F4级,即6人已经为肝硬化。总体看,吸毒人群肝纤维化损伤程度高于普通人群。结论 :抗病毒门诊丙肝者多,尤其集中在吸毒人群中,且存在急需治疗但未治疗患者,因此在着手加强社区尤其是吸毒人群中丙肝防治宣传,同时尽早为丙肝阳性感染者提供高效、低费用、高治愈率的丙肝抗病毒药物,从预防及治疗两面入手有效控制丙肝的传播。
Objective: To understand the dual infection of HIV and HCV in anti-virus treatment clinics in Mengzi City and the degree of liver injury in double infected patients. Methods: A cross-sectional survey was conducted to facilitate the sampling of HIV infected / AIDS patients receiving outpatient treatment in November 2014 as a survey subject and questionnaires to collect demographic data, hepatitis C infection, transglutaminase test, Aminotransferase test value, platelet test value and other information. Epidata was used to establish a database, FIB4 and APRI were used to evaluate liver fibrosis, and SPSS17.0 software was used for statistical analysis. Results: Totally 121 people were enrolled, of whom 38 were hepatitis C positive, of which 21 were from drug addicts. According to the APRI score, 28 had liver fibrosis, 2 had grade F2 or higher liver fibrosis and 5 had suspected liver hardening. According to the FIB-4 scoring method, 6 of the liver fibrosis was F3-F4 grade, that is, 6 were cirrhosis of the liver. Overall, drug-induced liver damage in liver cancer is higher than the general population. Conclusion: There are many hepatitis C patients with anti-virus clinics, especially those who are drug users, and there are patients who are in urgent need of treatment but not treated. Therefore, we are going to step up publicity on prevention and treatment of hepatitis C in the community, especially drug users, and at the same time provide hepatitis C positive patients with high efficiency and low Cost, high cure rate of hepatitis C antiviral drugs, prevention and treatment from both sides to effectively control the spread of hepatitis C.
