目的探讨影响环扎术治疗宫颈功能不全的影响因素。方法 2011年1月—2012年12月平度市人民医院收治宫颈功能不全患者55例,均给予宫颈环扎术治疗。计数资料采用χ2检验,P<0.05为差异有统计学意义。结果孕周14~18周手术成功率88.57%,无宫内感染;孕周18~25周手术成功率50.00%,宫内感染发生率10.00%。孕周14~18周手术成功率高于孕周18~25周,比较差异有统计学意义(P<0.05)。采用U形缝合手术成功率70.37%,宫内感染发生率3.70%;Mc Donald环扎手术成功率78.57%,宫内感染发生率3.57%。二者手术成功率、宫内感染发生率比较差异均无统计学意义(均P﹥0.05)。宫颈长度≥2.5 cm手术成功率72.50%,胎膜早破发生率5.00%;宫颈长度﹤2.5 cm手术成功率26.67%,胎膜早破发生率60.00%。缝合时宫颈长度≥2.5 cm手术成功率、胎膜早破发生率与宫颈长度﹤2.5 cm比较差异有统计学意义(P<0.05)。结论宫颈环扎术治疗宫颈功能不全时的孕周、缝合时宫颈长度与手术成功率有明显关系。
Objective To investigate the influencing factors of cerclage treatment of cervical insufficiency. Methods From January 2011 to December 2012, 55 patients with cervical insufficiency in Pingdu People’s Hospital were treated with cervical cerclage. Count data using χ2 test, P <0.05 for the difference was statistically significant. Results The gestational week 14 ~ 18 weeks surgical success rate of 88.57%, no intrauterine infection; gestational age 18 ~ 25 weeks surgical success rate was 50.00%, intrauterine infection rate of 10.00%. The gestational weeks 14 to 18 weeks surgical success rate was higher than gestational age 18 to 25 weeks, the difference was statistically significant (P <0.05). U-shaped suture operation success rate of 70.37%, 3.70% incidence of intrauterine infection; Mc Donald cerclage surgery success rate of 78.57%, intrauterine infection rate of 3.57%. The success rate of both surgery, intrauterine infection incidence difference was not statistically significant (P> 0.05). Cervical length ≥ 2.5 cm surgical success rate was 72.50%, the incidence of premature rupture of membranes was 5.00%; cervical length <2.5 cm operation success rate was 26.67%, the incidence of premature rupture of membranes was 60.00%. Cervical suture length ≥ 2.5 cm surgical success rate, the incidence of premature rupture of membranes and cervical length <2.5 cm, the difference was statistically significant (P <0.05). Conclusion Cervical cerclage treatment of cervical dysfunction at gestational age, suture cervical length and the success rate of surgery has a clear relationship.
