目的探讨床旁即时凝血检测仪(Coagu Chek XS)与常规检验科全自动凝血分析仪(Sysmex CA7000/CS5100)间INR结果的相关性与一致性。方法用Coagu Chek XS检测服用华法林患者的手指末梢血INR;用Sysmex CA7000/CS5100检测静脉血INR。对2种INR结果进行相关性分析和Kappa检验。结果入组49名患者,共获得111对INR。2种方法获得INR的平均差值为0.19±0.30。Coagu Chek XS与Sysmex CA7000/CS5100间INR结果有较好的相关性(r=0.970,P<0.001)和一致性(κ=0.646,P<0.001)。Sysmex CA7000/CS5100检测INR所需检测周转时间为(178.58±45.59)min,远远高于Coagu Chek XS检测INR所需检测周转时间。结论 2种检测方法具有较好的相关性与一致性。床旁即时凝血检测仪能节约检测周转时间,可以为服用华法林的患者提供监测INR的另一种选择。
Objective To investigate the correlation and agreement of INR between Coagu Chek XS and Sysmex CA7000 / CS5100 in routine laboratory. Methods Coagu Chek XS was used to detect peripheral blood INR of patients taking warfarin; venous blood INR was measured using Sysmex CA7000 / CS5100. Correlation analysis and Kappa test were performed on the two INR results. Results A total of 111 pairs of INRs were obtained in 49 patients. The mean INR difference between the two methods was 0.19 ± 0.30. There was a good correlation (r = 0.970, P <0.001) and concordance between Coagu Chek XS and Sysmex CA7000 / CS5100 (κ = 0.646, P <0.001). The test turnaround time required for Sysmex CA7000 / CS5100 to detect INR was (178.58 ± 45.59) min, much higher than the test turnaround time required for Coagu Chek XS to detect INR. Conclusion The two detection methods have good correlation and consistency. The Bedside Instant Coagulation Tester saves test turnaround time and provides an alternative to monitoring INR in patients taking warfarin.
