目的探讨MRI在脑囊虫病诊断中的价值。方法回顾性分析海口市人民医院2011年11月—2017年6月经病理或临床确诊的27例脑囊虫病患者的临床和MRI影像学资料,其中MRI增强24例,分析不同临床分型脑囊虫病的MRI影像表现。结果根据MRI影像学特点,27例脑囊虫病患者分成4型,以脑实质型最常见,共17例(63%),脑室型4例(15%),蛛网膜下腔型和混合型各3例(11%)。脑囊虫病分3期,其中活体期11例,变性死亡早期6例,变性死亡晚期10例。活体期典型MRI影像显示病灶中存在等信号点状头节,病灶周围无水肿,增强无强化;变性死亡早期仍可存在头节,病灶周围出现轻中度水肿,增强出现环形强化;变性死亡晚期的MRI所见头节消失。结论 MRI可以为脑囊虫病的诊断提供重要的影像学依据,有助于脑囊虫病临床分期及治疗。
Objective To investigate the value of MRI in the diagnosis of cerebral cysticercosis. Methods The clinical and MRI imaging data of 27 patients with cerebral cysticercosis diagnosed by pathology or clinical examination from November 2011 to June 2017 in Haikou People’s Hospital were retrospectively analyzed. Among them, MRI enhancement was performed in 24 cases and MRI of different clinical subgroups of cerebral cysticercosis Image performance. Results According to the characteristics of MRI, 27 patients with cysticercosis were divided into 4 types, the most common brain parenchyma type, 17 cases (63%), 4 cases (15%) in ventricular type, 3 subarachnoid and 3 Example (11%). Cerebral cysticercosis divided into three stages, of which 11 cases of life, early degeneration 6 cases of degeneration and death in 10 cases. Typical MRI images of living stage showed that there were isodal nodosities in the lesion with no edema around the lesion and enhanced no enhancement. In the early stage of degenerative death, head nodules could still exist. Mild and moderate edema appeared around the lesion and enhanced ring enhancement. Late degeneration MRI seen the first section disappears. Conclusion MRI can provide an important imaging basis for the diagnosis of cerebral cysticercosis and contribute to the clinical staging and treatment of cerebral cysticercosis.
