目的了解深圳市男男性行为人群(MSM)艾滋病病毒(HIV)感染状况及影响因素,寻找有效预防措施。方法采用时间场所抽样结合同伴推动抽样法招募MSM,开展问卷调查并采血检测HIV抗体。结果共调查707名MSM,平均年龄为(27.52±5.55)岁。样本来源于酒吧、歌舞厅、茶室、会所459人,占64.92%;网络招募212人,占29.99%;未婚630人,占89.11%;户籍在外市569人,占80.48%;性取向为同性恋504人,占71.29%。HIV感染率为10.18%。其中30岁组的感染率为17.03%,高于其它年龄组;性角色为“女性角色”的感染率(20.55%)高于“男性角色”(6.37%)和“男性女性角色均有”(10.23%)的感染率;最近六个月与同性发生肛交性行为时从未使用安全套的感染率(29.41%)高于有时使用安全套(8.75%)和每次都用安全套(8.57%)的感染率。单因素分析显示,MSM年龄、性角色、肛交性行为、使用安全套频率、商业性行为均与HIV感染有关(P均<0.05)。多因素分析显示,肛交时性角色为“女性角色”的人群最危险(OR=3.238,95%CI:1.397~7.508)。结论深圳市MSM人群HIV感染率较高,应加强宣传教育和行为干预,推广使用安全套。
Objective To understand the HIV status and influencing factors of MSM in Shenzhen and to find out the effective preventive measures. Methods The time-based sampling combined with companion-driven sampling method was used to recruit MSM to carry out questionnaires and blood tests to detect HIV antibodies. Results A total of 707 MSM were investigated, with an average age of (27.52 ± 5.55) years. Sample comes from bars, dance halls, tea rooms, clubs, 459 people, accounting for 64.92%; network recruit 212 people, accounting for 29.99%; unmarried 630 people, accounting for 89.11%; 569 people in other cities, accounting for 80.48%; sexual orientation is homosexual 504 People, accounting for 71.29%. HIV infection rate was 10.18%. The infection rate of 30-year-old group was 17.03%, higher than that of other age groups. The infection rate of sexual role as female role was higher than that of male role (20.55%) and male gender (10.23%). The rate of never using condoms during homosexual anal sex in the last six months (29.41%) was higher than for condoms (8.75%) and condoms (8.57%) infection rate. Univariate analysis showed that MSM age, sexual role, anal sex, frequency of using condoms and commercial sex were all related to HIV infection (all P <0.05). In the multivariate analysis, the most dangerous group was “female character” (OR = 3.238, 95% CI: 1.397-7.508). Conclusion HIV prevalence in MSM population is high in Shenzhen. Publicity and education and behavioral intervention should be strengthened to promote the use of condoms.
