目的:检测正常妊娠和妊娠期糖尿病(GDM)孕妇在中期妊娠时视黄醇结合蛋白和游离脂肪酸的血清水平,探讨妊娠妇女血清视黄醇结合蛋白和游离脂肪酸早期预测(GDM)的价值。方法采用病例对照研究,检测并比较88例GDM及88例正常妊娠妇女孕16~20周血清视黄醇结合蛋白和游离脂肪酸浓度水平。结果 GDM组血清视黄醇结合蛋白和游离脂肪酸浓度均高于对照组(P<0.05),血清视黄醇结合蛋白和游离脂肪酸水平呈正相关(r=0.336,P<0.01)。血清视黄醇结合蛋白≥30.45 mg/L时,预测(GDM)的敏感性为63.6%,特异性为75.0%;血清游离脂肪酸≥0.575 mmol/L时,预测(GDM)的敏感性为56.8%,特异性为63.6%。结论孕16~20周孕妇血清视黄醇结合蛋白水平对于(GDM)的预测价值高于血清游离脂肪酸。
Objective: To detect serum levels of retinol binding protein and free fatty acid in pregnant women with normal pregnancy and gestational diabetes mellitus (GDM) in the second trimester of pregnancy and to explore the value of serum retinol binding protein and free fatty acid prediction (GDM) in pregnant women. Methods A case-control study was conducted to detect and compare serum retinol binding protein and free fatty acid levels in 88 GDM pregnant women and 88 normal pregnant women between 16 and 20 weeks of gestation. Results Serum retinol binding protein and free fatty acid concentrations in GDM group were significantly higher than those in control group (P <0.05). Serum retinol binding protein and free fatty acid levels were positively correlated (r = 0.336, P <0.01). The sensitivity of prediction (GDM) was 63.6% and the specificity was 75.0% when serum retinol binding protein was ≥30.45 mg / L, and the sensitivity of prediction (GDM) was 56.8% when serum free fatty acid was ≥0.575 mmol / L , Specificity of 63.6%. Conclusion The predictive value of serum retinol binding protein (GDM) in pregnant women with gestational age from 16 to 20 weeks is higher than that of serum free fatty acids.
