目的 探讨剖宫产瘢痕子宫再次妊娠的合理分娩方式,瘢痕子宫足月妊娠阴道分娩的可行性,降低剖宫产率、降低母婴远近期并发症.方法 对190例瘢痕子宫足月妊娠孕妇的分娩方式进行回顾性分析.结果 190例瘢痕子宫足月妊娠孕妇中阴道试产128例,阴道分娩102例,阴道试产成功率约79.7%,其中自然产89例,阴道助产13例,未发生子宫破裂,出血量平均(132.5±20.3)ml,新生儿Apgar评分(9.26±0.2)分,再次剖宫产62例,其中41例存在绝对剖宫产指征,21例拒绝试产,出血量(387.0±48.2)ml,新生儿Apgar评分(9.02±0.2)分.结论 瘢痕子宫不是剖宫产的绝对指征.符合阴道分娩试产条件者在严密连续监护下经阴道分娩是完全可行的.
