目的:探讨左心室肥厚(LVH)及脉压(PP)与老年原发性高血压(EH)患者并发脑梗死之间的关系及其临床检测意义。方法:使用超声方法检测52例老年并发脑梗死的EH患者及45例老年非并发症脑梗死的EH患者左心室质量指数(LVMI),结合患者血压及PP情况进行比较和综合分析。结果:并发脑梗死组的LVMI及PP显著高于非脑梗死组,其差异有统计学意义(P<0.01)。并且LVMI随PP增加而增加,LVMI与PP呈显著正相关,脑梗死组和非脑梗死组LVMI与PP相关系数分别为0.632和0.645(P<0.01),所有患者LVMI与PP之间相关系数为0.360(P<0.01)。结论:脑梗死患者LVMI及PP显著增加,且LVMI与PP呈显著正相关。同时监测LVMI和PP有助于了解老年EH患者疾病的不同状态,便于判断病程及预后。
Objective: To investigate the relationship between left ventricular hypertrophy (LVH), pulse pressure (PP) and senile essential hypertension (EH) complicated with cerebral infarction and its clinical significance. Methods: The left ventricular mass index (LVMI) was measured in 52 elderly EH patients with cerebral infarction and 45 elderly patients with EH complicated with cerebral infarction. The blood pressure and PP were compared and analyzed comprehensively. Results: LVMI and PP in patients with cerebral infarction were significantly higher than those in non-cerebral infarction patients (P <0.01). And LVMI increased with the increase of PP, LVMI and PP was positively correlated, LVMI and PP in cerebral infarction group and non-cerebral infarction group were 0.632 and 0.645 respectively (P <0.01). The correlation coefficient between LVMI and PP was 0.360 (P <0.01). Conclusion: The LVMI and PP were significantly increased in patients with cerebral infarction, and there was a significant positive correlation between LVMI and PP. Simultaneous monitoring of LVMI and PP helps to understand the different states of disease in elderly patients with EH, to facilitate the assessment of disease duration and prognosis.
