目的 :探讨特发性血小板减少性紫癜 (ITP)患儿的细胞免疫与体液免疫功能的变化。方法 :用流式细胞术 (FCM )与免疫散射速率比浊法分别测定 2 0例ITP患儿的淋巴细胞亚群的分布、免疫球蛋白 (IgG ,IgA和IgM)和补体 (C3 ,C4)的水平。结果 :在治疗前 ,ITP患儿的CD3 + ,CD4+ 和CD4+ /CD8+ 比值、NK细胞均下降 ,CD8+ 升高 ,免疫球蛋白亦降低 ,对照组及恢复组比较有显著性差异 (P <0 .0 5 ) ,而B淋巴细胞 (CD9+)、补体(C3 ,C4)没有明显的变化 ;在治疗后 ,血小板恢复正常 (>10 0× 10 9/L)时 ,ITP患儿的CD3 + ,CD4+ 和CD4+ /CD8+ 比值、NK细胞、免疫球蛋白均回升至正常 ,CD8+ 下降 ,与治疗前比较有显著性差异 (P <0 .0 5 )。结论 :ITP患儿存在细胞免疫与体液免疫的紊乱 ,在治疗上应着眼纠正免疫紊乱。
Objective: To investigate the changes of cellular immunity and humoral immunity in children with idiopathic thrombocytopenic purpura (ITP). Methods: The distribution of lymphocyte subsets, immunoglobulins (IgG, IgA and IgM) and complement (C3, C4) in 20 children with ITP were determined by flow cytometry (FCM) and immunostaining rate nephelometry s level. Results: Before treatment, the ratio of CD3 +, CD4 + and CD4 + / CD8 + in ITP children decreased, NK cells decreased, CD8 + increased and immunoglobulin decreased. There was significant difference between control group and recovery group (P <0. There was no significant change in B lymphocytes (CD9 +) and complement (C3, C4). After treatment, platelet count returned to normal (> 10 × 10 9 / L) And CD4 + / CD8 + ratio, NK cells and immunoglobulin returned to normal, while CD8 + decreased. There was a significant difference between before and after treatment (P <0.05). Conclusion: ITP children with cellular immunity and humoral immunity disorders, should focus on the treatment to correct immune disorders.
