目的了解2009年海南省预防接种异常反应的发生特征,评价海南省2009年疑似预防接种异常反应(AdverseEvents Following Immunization,AEFI)监测系统运行效果,为及时正确处理疑似预防接种异常反应,降低严重异常反应的发生率,防止接种事故的发生提供科学依据。方法接种单位按照海南省AEFI监测方案,通过监测网络系统报告AEFI专报系统,县级完成个案调查并通过异常反应调查诊断小组进行调查诊断和处理。采用描述流行病学方法 ,对上报的AEFI监测数据和处理进行分析和评价。结果 2009年共报告AEFI个案609例,其中一般反应485例,占报告总数的79.6%;异常反应106例,占17.4%;其他不良反应18例,占3%。AEFI报告县级覆盖率为100%,乡级单位覆盖率为62%。报告及时率87.33%,调查及时率96.4%。一类疫苗报告发生率为1.32/万剂,居前3位的疫苗是全细胞百白破、卡介苗和无细胞百白破疫苗,分别为4.53/万剂、2.80/万剂、1.89/万剂。临床损害排前3位的是过敏性反应、卡介苗淋巴结炎和无菌性脓肿。结论 2009年海南省AEFI网络监测系统运转正常,监测敏感性提高。
Objective To understand the occurrence characteristics of immunization in Hainan Province in 2009 and evaluate the effect of surveillance system of Hainan Province in 2009 on suspected antivirus immunization (AEFI). Methods According to the AEFI monitoring plan of Hainan Province, the vaccination unit reported the AEFI special report system through the monitoring network system, completed the case investigation at the county level, and investigated and diagnosed the cases by the abnormal response investigation diagnosis team. Descriptive epidemiological methods were used to analyze and evaluate reported AEFI surveillance data and treatments. Results A total of 609 AEFI cases were reported in 2009, of which 485 cases were general reactions, accounting for 79.6% of the total. 106 cases were abnormal reactions, accounting for 17.4%. Other adverse reactions were 18 cases (3%). AEFI reports 100% coverage at the county level and 62% coverage at the township level. The report promptly rate 87.33%, the survey promptly rate 96.4%. The incidence of one type of vaccine was 1.32 per 10,000 doses. The top three vaccines were whole-cell-diphtheria, BCG and cell free diphtheria-baicalein vaccine, 4.53 / 10 000, 2.80 / 10000 and 1.89 / . Top 3 clinical lesions are allergic reactions, BCG lymphadenitis and aseptic abscess. Conclusion The AEFI network monitoring system in Hainan Province was operating normally in 2009, and the sensitivity of monitoring increased.
