目的评价封堵器经导管介入在治疗动脉导管未闭(PDA)、房间隔缺损(ASD)及室间隔缺损(VSD)中的疗效与安全性。方法回顾性分析本院2000年1月至2005年6月应用封堵器经导管介入治疗的PDA、ASD及VSD患者55例临床资料。55例先天性心脏病患者(PDA33例,ASD16例,VSD6例)均在X线透视及造影和超声心动图监视下经导管置入封堵器治疗PDA、ASD及VSD。术后分别于1~3 d复查超声心动图,随访1个月~3年。结果PDA封堵成功率94%,ASD封堵成功率88%,VSD封堵成功率100%。封堵成功的患者均于术后24h、1个月~3年行经胸超声心动图检查和随访,均无残余分流和并发症,右心房、右心室内径均有不同程度的缩小,左、右心室功能得到了改善,生活质量良好。结论经导管置入封堵器是治疗PDA、ASD和VSD的一种操作简便、技术成功率和安全性高、创伤小、并发症少、疗效可靠的治疗方法,应用封堵器介入治疗应该成为PDA、ASD及VSD治疗的首要选择。
Objective To evaluate the efficacy and safety of occluder catheterization in the treatment of patent ductus arteriosus (PDA), atrial septal defect (ASD) and ventricular septal defect (VSD). Methods A retrospective analysis of clinical data of 55 cases of PDA, ASD and VSD patients treated with catheter occluder in our hospital from January 2000 to June 2005 was retrospectively analyzed. 55 cases of congenital heart disease (PDA33 cases, ASD16 cases, VSD6 cases) underwent X-ray and contrast and echocardiographic monitoring by PDA catheter, ASD and VSD. Echocardiography was performed at 1 to 3 days after operation and was followed up for 1 month to 3 years. Results PDA closure success rate of 94%, ASD closure success rate of 88%, VSD closure success rate of 100%. Patients with successful closure had transthoracic echocardiography and follow-up at 24 h, 1 month to 3 years after operation. There were no residual shunts and complications. The diameters of right atrium and right ventricle were reduced to varying degrees. Left and right Ventricular function has been improved, good quality of life. Conclusion Transcatheter occlusion of occluder is a simple and convenient method for the treatment of PDA, ASD and VSD, with high technical success rate and safety, less trauma, less complications and reliable curative effect. The application of occluder intervention should become PDA, ASD and VSD treatment of the first choice.
