目的:探讨脂肪乳剂在梗阻性黄疸患者围手术期营养支持中的应用,以及对肝功能的影响。方法:将患者随机分为对照组(A=13例)和脂肪乳剂组((B=10例)),比较两组手术前后丙氨酸转氨酶(ALT)、血总胆红质(TBIL)、血白蛋白(ALB)、血清前白蛋白(PAB)的变化。数据处理均采用t检验和配对t检验。结果:两组术后ALT、TBIL均明显下降(A组P<0.01,B组P<0.05),两组术后ALB均低于术前(P<0.05)。PAB∶A组,术后明显降低,与术前相比差异有显著性(P<0.05);B组,术后有所升高,但差异无显著性(P>0.05)。结论:阻黄患者胆汁引流术后,使用脂肪乳剂可以发挥其补充能源、节省氮源的作用,且有利于肝功能的恢复。
Objective: To investigate the application of fat emulsion in perioperative nutritional support of patients with obstructive jaundice and its effect on liver function. Methods: The patients were randomly divided into control group (A = 13 cases) and fat emulsion group (B = 10 cases). The levels of ALT, TBIL, Serum albumin (ALB), prealbumin (PAB) changes. Data processing using t test and paired t test. Results: ALT and TBIL decreased significantly in both groups (P <0.01 in group A, P <0.05 in group B). The postoperative ALB levels in both groups were significantly lower than those before operation (P <0.05). PAB: A group, postoperative was significantly lower, compared with the preoperative difference was significant (P <0.05); B group, postoperative increased, but no significant difference (P> 0.05). Conclusion: After bile drainage in patients with obstruction of yellow, the use of fat emulsion can play its complementary energy, save the role of nitrogen sources, and is conducive to the recovery of liver function.
