目的探讨HPV E6/E7mRNA检测在宫颈病变诊断中的应用价值。方法选取2013年1—6月液基细胞学检测为ASCUS的患者130例,行组织病理学检查,同时应用b-DNA技术对组织学蜡块进行HPV E6/E7mRNA检测。结果 HPV E6/E7mRNA检测低危风险24例(病理组织学正常1例,CIN1级23例);中危风险22例(CIN1级9例,CIN2级13例),高危风险11例(CIN2级4例,CIN3级6例,宫颈鳞癌1例),阳性率43.8%,病理组织学诊断CIN2级以上病变24例,相应HPV E6/E7mRNA检测均为中危以上风险。经统计学分析,差异有统计学意义(P<0.05)。结论 HPV E6/E7mRNA含量可以更好地反应致癌基因的活动程度,具有更高的敏感性及特异性,可以直观的评估宫颈癌的发病风险及预后,同时对于组织学阴性或较低级别的病理组织学起到预测疾病发生的作用。
Objective To investigate the value of HPV E6 / E7 mRNA in the diagnosis of cervical lesions. Methods One hundred and thirty patients with ASCUS detected by liquid-based cytology from January to June in 2013 were selected for histopathological examination and HPV E6 / E7 mRNA was detected by b-DNA technique. Results The low risk of HPV E6 / E7 mRNA was detected in 24 cases (1 case of normal histopathology and 23 cases of CIN1 grade), 22 cases of intermediate risk (9 cases of CIN1 and 13 cases of CIN2) and 11 cases of high risk (CIN2 4 6 cases of CIN3 grade and 1 case of cervical squamous cell carcinoma), the positive rate was 43.8%. The histopathological diagnosis of CIN2 and above lesions were 24 cases. The corresponding HPV E6 / E7 mRNA levels were all above mid-risk. The statistical analysis, the difference was statistically significant (P <0.05). Conclusion HPV E6 / E7 mRNA level can better reflect the activity of oncogenes with higher sensitivity and specificity, and can directly assess the risk and prognosis of cervical cancer. Meanwhile, for histological negative or lower pathological Histology plays a role in predicting the onset of disease.
