目的探讨微创手术治疗急性重型胰腺炎的疗效。方法根据治疗方案将2011年3月至2014年2月安阳地区医院收治的102例急性重型胰腺炎患者分为观察组(55例)与对照组(47例),观察组接受微创手术治疗,对照组接受奥曲肽联合乌司他丁治疗,对比分析两组患者的疗效。结果治疗前,两组患者血清淀粉酶(AMY)、尿素氮(BUN)、肌酐(Cr)、血糖(GLU)、血钙(Ca)比较差异未见统计学意义(P>0.05)。治疗后,观察组血清AMY、BUN、Cr、GLU显著低于对照组,Ca显著高于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者白细胞计数(WBC)、中性粒细胞比值(NEU)、动脉血酸碱度(p H)、动脉血氧分压(Pa O2)比较差异未见统计学意义(P>0.05)。治疗后,观察组WBC、NEU显著低于对照组,p H、Pa O2显著高于对照组,差异有统计学意义(P<0.05)。观察组腹痛持续时间、腹胀持续时间、腹膜刺激征持续时间显著低于对照组,病死率显著低于对照组,差异有统计学意义(P<0.05)。结论微创手术治疗急性重型胰腺炎的疗效不容忽视,值得临床推广应用。
Objective To investigate the curative effect of minimally invasive surgery on acute severe pancreatitis. Methods According to the treatment plan, 102 patients with acute severe pancreatitis admitted to Anyang District Hospital from March 2011 to February 2014 were divided into observation group (55 cases) and control group (47 cases). The observation group received minimally invasive surgery, Control group received octreotide combined with ulinastatin treatment, comparative analysis of two groups of patients. Results Before treatment, the levels of serum amylase (AMY), blood urea nitrogen (BUN), creatinine (Cr), blood glucose (GLU) and calcium in the two groups had no significant difference (P> 0.05). After treatment, serum AMY, BUN, Cr and GLU in the observation group were significantly lower than those in the control group, Ca was significantly higher than the control group, the difference was statistically significant (P <0.05). Before treatment, the WBC, NEU, p H and Pa O2 of the two groups had no significant difference (P> 0.05) . After treatment, the WBC and NEU in the observation group were significantly lower than those in the control group, and the levels of p H and Pa O2 in the observation group were significantly higher than those in the control group (P <0.05). The duration of abdominal pain, the duration of abdominal distension and the duration of peritoneal irritation in the observation group were significantly lower than those in the control group, and the mortality was significantly lower than that in the control group (P <0.05). Conclusions The curative effect of minimally invasive surgery on acute severe pancreatitis should not be neglected, which is worthy of clinical application.
