目的评价两种龈下冲洗液使用前后慢性牙周炎龈沟液前列腺素E2(PGE2)和白细胞介素-6(IL-6)水平的变化,为临床使用提供实验依据。方法选取慢性牙周炎患者30例,随机分为甲硝唑注射液组、3%双氧水组和对照组,每组10例,每人选择6颗指数牙。全口洁治后使用滤纸条取所有指数牙的龈沟液。然后分别用甲硝唑注射液和3%生理盐水对指数牙进行龈下冲洗,每周一次,对照组不进行龈下冲洗,4周后再次使用滤纸条取所有指数牙龈沟液。采用ELISA检测冲洗前后3组龈沟液PGE2和IL-6的含量。结果龈下冲洗4周后甲硝唑组龈沟液PGE2和IL-6比龈下冲洗前明显减少,且甲硝唑组的作用优于双氧水组,而对照组4周前后PGE2和IL-6含量没有明显变化。结论两种龈下冲洗液都能够减少龈沟液PGE2和IL-6的水平,从而改善牙周组织的炎症状态,减缓牙槽骨的吸收。
Objective To evaluate the changes of prostaglandin E2 (PGE2) and interleukin-6 (IL-6) levels in gingival crevicular fluid of patients with chronic periodontitis before and after the use of two subgingival washings, and to provide experimental evidence for clinical use. Methods Thirty patients with chronic periodontitis were randomly divided into metronidazole injection group, 3% hydrogen peroxide group and control group, with 10 patients in each group, each with 6 index teeth. After cleaning all the mouth using the filter paper to take all the index teeth of the gingival crevicular fluid. The index teeth were then sub-irrigated with metronidazole injection and 3% saline once a week, while the control group without subgingival irrigation. After 4 weeks, all index gingival crevicular fluid was removed using filter paper. The levels of PGE2 and IL-6 in three groups of gingival crevicular fluid before and after washing were detected by ELISA. Results After 4 weeks of subgingival irrigation, the levels of PGE2 and IL-6 in gingival crevicular fluid of metronidazole group were significantly lower than those before subgingival irrigation, and the effect of metronidazole group was better than that of hydrogen peroxide group. However, the content of PGE2 and IL-6 obvious change. Conclusion Both subgingival irrigation solutions can reduce the levels of PGE2 and IL-6 in gingival crevicular fluid, thereby improving the inflammatory status of periodontal tissues and alleviating the alveolar bone absorption.
