目的:探讨妊娠期合并急性肾损伤孕妇子宫动脉及胎儿脐动脉血流动态变化规律及其对妊娠结局的影响。方法:采用回顾性研究的方法,选取2009年2月~2010年3月在该院行超声检查的130例孕产妇作为研究对象,其中妊娠合并急性肾损伤50例(高危组),正常妊娠80例,两组分别在不同妊娠时期监测母体子宫动脉、脐动脉的血流动力学变化,并观察妊娠结局。结果:高危组中子宫动脉和脐动脉的S/D值、阻力指数(RI)、搏动指数(PI)等指标均显著高于正常妊娠组;高危组的分娩孕周、胎心异常率、剖宫产率及新生儿的出生体重、胎盘重量和窒息率与正常妊娠组比较均有统计学差异。结论:子宫动脉和脐动脉血流动力学指标可作为妊娠期的一项无创性检查方法,其对不良妊娠结局的预测有较大的应用价值。
Objective: To investigate the dynamic changes of blood flow in uterine artery and fetal umbilical artery in pregnant women with acute renal injury and its effect on pregnancy outcome. Methods: A retrospective study was conducted to select 130 pregnant women undergoing ultrasound examination from February 2009 to March 2010 in our hospital. Among them, 50 pregnant women with acute renal injury (high risk group), 80 normal pregnant women For example, the two groups monitored the changes of hemodynamics of maternal uterine artery and umbilical artery in different gestation periods, and observed the pregnancy outcome. Results: The S / D value, RI, PI of high uterine artery and umbilical artery in high risk group were significantly higher than those in normal pregnancy group. The gestational age, abnormal fetal heart rate, Palace birth rate and newborn birth weight, placental weight and asphyxia rate compared with the normal pregnancy group were statistically significant. Conclusion: The uterine artery and umbilical artery hemodynamic parameters can be used as a noninvasive method during pregnancy, which has great value in predicting adverse pregnancy outcome.
