目的对临床患者首次进行输注前血小板交叉配型进行分析研究。方法 2010年6月-2016年3月在北京市通州区中心血站首次进行血小板交叉配型试验的所有患者,每位患者分别与随机选取的5名ABO同型献血者捐献的血小板进行交叉配型试验,有阴性、弱阳性和阳性3种试验结果,利用统计学软件进行结果分析。结果共统计了276例患者首次进行血小板进行交叉配型试验的1 380个配型试验,其中配合血小板只有317份,配合率22.97%。男性患者首次交叉配型试验配合血小板所占比例(25.60%)高于女性患者(19.84%),(P<0.05)。不同血型组血小板交叉配型试验结果无显著差异(P>0.05)。不同年龄组血小板交叉配型试验结果有显著差异(P<0.05),60岁以上的老年患者的不配合率(79.76%)高于其他年龄组,且平均配型次数也多于其他年龄组。不同疾病组血小板交叉配型试验结果存在显著差异(P<0.05),再生障碍性贫血患者首次配型阴性结果比例(18.9%)低于其他疾病组。结论不同性别、年龄、疾病患者的首次血小板交叉配型试验结果有差异,临床应根据患者情况及早进行输注前血小板交叉配型,以提高血小板阴性配合率,便于患者输注相合血小板提高输注疗效。
Objective To analyze and analyze the first transfusion platelet crossmatch in clinical patients. Methods From June 2010 to March 2016, all patients with the first platelet crossmatch test in Tongzhou District Blood Center of Beijing were crossed with platelets respectively donated by five randomly selected ABO donors Test, negative, weak positive and positive three kinds of test results, the use of statistical software for analysis of the results. Results A total of 1 380 matching tests were conducted in 276 patients for the first time with platelet crossmatching test. There were only 317 matching platelets, with a rate of 22.97%. The proportion of male patients with cross matching test with platelet (25.60%) was higher than that of female patients (19.84%) (P <0.05). There was no significant difference in platelet crossmatch test between different blood groups (P> 0.05). There were significant differences in platelet crossmatch test between different age groups (P <0.05). The mismatch rate of elderly patients over 60 years old (79.76%) was higher than that of other age groups, and the average number of matching was also more than that of other age groups. There was a significant difference in platelet crossmatch test results between different disease groups (P <0.05), and the proportion of first matched negative results (18.9%) in patients with aplastic anemia was lower than in other disease groups. Conclusions The results of the first platelet crossmatch test in patients with different gender, age and disease are different. According to the patient’s condition, the pre-transfusion platelet crossmatch should be used as early as possible to improve the negative rate of platelet transfusion and facilitate the infusion of matched platelets in patients Efficacy.
