心肺运动试验(CPET)为一种诊断手段,通过测量运动时肺内气体交换,同步评估心血管和呼吸系统对运动的应激反应。CPET是了解心血管和呼吸系统病理生理的一种最廉价的诊断方法,与其他诊断性试验只评价单一器官系统不同,CPET可同步评价运动相关的各器官系统的功能。目前CPET已经成为一种重要的临床检测工具,广泛应用于运动耐量的评估,外科手术前风险评估,心力衰竭严重程度分级、疾病治疗效果,鉴别呼吸困难等领域。侵入性CPET增加了肺动脉和桡动脉置管,可测定血流动力学参数和血气分析,用于鉴别常用方法不能诊断的亚临床性疾病包括运动诱发的肺动脉高压、早期的舒张性心力衰竭、前负荷不足、线粒体肌病等导致的运动耐量受损等。
Cardiopulmonary exercise testing (CPET) is a diagnostic tool for simultaneous assessment of cardiovascular and respiratory stress responses to exercise by measuring intrapulmonary gas exchange during exercise. CPET is one of the cheapest diagnostic methods for understanding the cardiovascular and respiratory pathophysiology. Unlike other diagnostic tests evaluating only a single organ system, CPET can simultaneously assess the function of various organ systems related to exercise. Currently, CPET has become an important clinical testing tool widely used in the fields of exercise tolerance assessment, pre-surgery risk assessment, severity of heart failure classification, disease treatment effect, differential diagnosis of dyspnea and other fields. Intrusive CPET increases the placement of pulmonary and radial arteries, measures hemodynamic parameters and blood gas analysis, and identifies subclinical conditions that can not be diagnosed by common methods, including exercise-induced pulmonary hypertension, early diastolic heart failure, Under-load, mitochondrial myopathy and other exercise-induced damage.
