目的探讨非胰岛素依赖型糖尿病(non-insulin-dependentdiabetesmelitus,NIDDM)患者的糖尿病视网膜病变(diabeticretinopathy,DR)与脂质代谢紊乱的关系。方法对有DR的NIDDM患者60例,无DR的NIDDM患者75例以及正常对照55例,检测了血浆总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)及其亚类,以及空腹血糖(FPG)、空腹血浆胰岛素(FINS)和糖基化血红蛋白(HbA1C)。同时按公式计算血浆低密度脂蛋白(LDL)和极低密度脂蛋白(VLDL)。结果DR组的TC、LDL、FPG、HbA1C及病程高于或长于非DR组。在DR患者中,增殖型DR患者的TC、LDL、FPG、FINS、HbA1C及病程高于或长于单纯型DR患者。相关分析表明,DR患者的病变严重程度与TC、LDL、HbA1C和病程呈显著正相关。结论脂质代谢紊乱在NIDDM患者的DR发生、发展中,可能起着一定的作用。
Objective To investigate the relationship between diabetic retinopathy (DR) and dyslipidemia in non-insulin-dependent diabetes mellitus (NIDDM). Methods Sixty patients with NIDDM with DR, 75 with NIDDM without DR, and 55 normal controls were enrolled in this study. Plasma total cholesterol (TC), triglyceride (TG), high density lipoprotein (HDL-C) Subclasses, as well as fasting plasma glucose (FPG), fasting plasma insulin (FINS) and glycosylated hemoglobin (HbA1C). Meanwhile, plasma LDL and VLDL were calculated according to the formula. Results The levels of TC, LDL, FPG and HbA1C in DR group were higher than those in non-DR group. In DR patients, TC, LDL, FPG, FINS, HbA1C in patients with proliferative DR are higher or longer than patients with simple DR. Correlation analysis showed that the severity of the disease in DR patients was positively correlated with TC, LDL, HbA1C and duration of disease. Conclusion The disorder of lipid metabolism may play a role in the occurrence and development of DR in NIDDM patients.
