目的:探讨n 18F-脱氧葡萄糖(FDG)PET/MR在肝转移瘤诊断中的应用价值。n 方法:回顾性分析2020年1月至2020年10月间于瑞金医院接受PET/MR与上腹部增强CT检查的可疑肝转移瘤患者75例[男46例,女29例;年龄(58.9±14.3)岁],分别基于病灶和患者计算并比较增强CT、PET、MR与PET/MR诊断肝转移瘤的效能(McNemar检验)。结果:75例患者共检出肝转移瘤病灶306处,其中45例(179处病灶)随访或病理证实为肝转移瘤。基于病灶的分析显示,增强CT、PET、MR和PET/MR的灵敏度分别为74.9%(134/179)、60.3%(108/179)、98.9%(177/179)和100%(179/179),特异性分别为96.9%(123/127)、100%(127/127)、92.9%(118/127)和92.1%(117/127)。PET/MR的诊断效能高于增强CT和PET(n χ2值:51.000和81.000,均n P<0.001),但与MR相比差异无统计学意义(n χ2=2.000,n P=0.368)。基于患者的分析显示,增强CT、PET、MR和PET/MR的灵敏度分别为82.2%(37/45)、84.4%(38/45)、95.6%(43/45)和100%(45/45),特异性分别为86.7%(26/30)、100%(30/30)、70.0%(21/30)和70.0%(21/30)。PET/MR与增强CT和PET的诊断效能差异均有统计学意义(n χ2值:13.000和16.000,均n P<0.05),但MR相比差异无统计学意义(n χ2=2.000,n P=0.368)。n 结论:18F-FDG PET/MR较单模态显像具有更高的肝转移瘤检出率;其整体诊断肝转移瘤的性能优于增强CT和单纯PET,但与MR相比无明显优势。n “,”Objective:To explore the diagnostic value of n 18F-fluorodeoxyglucose (FDG) PET/MR imaging for liver metastasis.n Methods:A retrospective analysis of 75 cases (46 males, 29 females; age (58.9±14.3) years) with suspected liver metastases from January 2020 to October 2020 in Ruijin Hospital were performed. All patients underwent PET/MR and enhanced upper abdominal CT scans. Diagnostic efficacies of enhanced CT, PET, MR and PET/MR for liver metastases (based on lesions and patients respectively) were calculated and compared (McNemar test).Results:A total of 306 liver lesions were detected in 75 patients, of which 179 lesions in 45 patients were confirmed as liver metastases through follow-up or pathology. In lesion-based analysis, the sensitivities of enhanced CT, PET, MR and PET/MR were 74.9%(134/179), 60.3%(108/179), 98.9%(177/179) and 100%(179/179), with specificities of 96.9%(123/127), 100%(127/127), 92.9%(118/127) and 92.1%(117/127), respectively. The diagnostic efficacy of PET/MR was significantly higher than that of enhanced CT and PET (n χ2 values: 51.000 and 81.000, both n P<0.001), but there was no statistical difference between PET/MR and MR (n χ2=2.000, n P=0.368).
