目的探讨不同类型巨大裂孔视网膜脱离的手术适应症、手术方式及眼内填充物的选择。方法回顾1991年1月～1995年6月间我院治疗的巨大裂孔视网膜脱离病例108例108眼。结果术后平均随访期36月，手术总成功率为60．19％（65／108），环扎加压术成功率51．02％（25／49），玻璃体手术67.80％（40／59）。对赵氏膜分级Ⅱ级以上，巨大裂孔大于180°和复杂型巨大裂孔，玻璃体手术成功率明显高于常规视网膜脱离手术。结论（1）对裂孔≤120°赵氏膜分级Ⅰ、Ⅱ级的单纯性巨大裂孔，可选择常规视网膜脱离手术，而对于其他类型的巨大裂孔，特别是裂孔≥180°，赵氏膜Ⅱ级和复杂型巨大裂孔，应首选玻璃体手术；（2）玻璃体手术眼内填充物应首选C3F8气体。
Objective To investigate the surgical indications, surgical methods and the choice of intraocular filling for different types of huge retinal detachment. Methods From January 1991 to June 1995, 108 cases of 108 cases of giant retinal detachment were treated in our hospital. Results The mean follow-up period was 36 months (60.19%, 65.02%). The success rate of cerclage surgery was 51.02% (25/49), and vitreous surgery was 67.80% (40/59) . On the Zhao film grade Ⅱ or more, a large hole greater than 180 ° and complex huge hole, vitreous surgery was significantly higher than the success rate of conventional retinal detachment surgery. Conclusions (1) For simple giant hiatus with grade Ⅰ and Ⅱ of grade ≤120 °, the conventional retinal detachment surgery can be selected. For other types of giant hiatus, especially with ≥180 °, And complex huge hole, should be the preferred vitreous surgery; (2) vitreous surgery intraocular filling should be the preferred C3F8 gas.
