Objective To study the balance of Th1 / Th2 cells in synovial fluid and peripheral blood of patients with rheumatoid arthritis (RA) at a single cell level and its relationship with disease activity. Methods The expressions of intracellular cytokines IFN-γ and IL-4 in synovial fluid mononuclear cells (SFMCs) and peripheral blood mononuclear cells (PBMCs) were detected by ELISPOT. Results The ratio of Th1 and Th1 / Th2 in SFMCs in RA patients was significantly higher than that in PBMCs of patients with and without RA (P <0.01), and the Th1 / Th2 ratio was positively correlated with Stoke index (r = 0 893, P 0 00 1). Conclusions The pattern of cytokine secretion toward Th1 in RA patients is biased towards Th1. Th1 / Th2 ratio is associated with disease activity. Th1 / Th2 balance plays an important role in the pathogenesis and progression of RA.
