目的通过透明牙标本法、锥体束CT扫描法,研究山东地区人下颌第一前磨牙根管系统的形态。方法从山东各地市医院口腔科选取符合条件的离体下颌第一前磨牙共计164颗为研究对象,肉眼观察其牙根外形,记录有根面凹陷牙齿的数量,通过透明牙标本法采用Vertuccis分类法观察根管系统形态。选取山东大学口腔医院放射科的锥体束CT(cone-beam CT,CBCT)影像资料中304颗山东人下颌第一前磨牙的影像,观察根管系统形态。结果透明牙标本法中,164颗离体牙,有2颗为双根牙,162颗为单根牙,双根牙检出率为1.22%;162颗单根牙的根管中,Ⅰ型根管检出率82.72%,Ⅲ型根管检出率为1.23%,Ⅳ型根管检出率为3.09%,Ⅴ型根管检出率为12.96%,Ⅱ、Ⅵ、Ⅶ、Ⅷ型根管检出率为0。164颗离体牙透明牙标本法的双根管检出率为18.29%;36颗离体牙有根面凹陷,其中21颗牙根管数目为单根管,其余15颗为双根管,有根面凹陷的离体牙双根管检出率41.67%;锥体束CT扫描304颗下颌第一前磨牙,55颗牙为双根管,双根管检出率为18.09%。结论山东地区人下颌第一前磨牙双根管出现率比较高,根管形态比较复杂,临床治疗时需注意,可采取多种辅助手段,确定根管形态。
Objective To study the morphology of the root canal system of the human premolars of the mandibular first molar in Shandong Province by means of transparent dental specimens and pyramidal tract computed tomography. Methods A total of 164 qualified first premolars of mandibular first mandibular molar were selected from Department of Stomatology, Shandong Provincial Hospital, Shandong Province, China. Results In the method of transparent tooth, 164 teeth were isolated, 2 teeth were double root, 162 teeth were single root, the detection rate of double root was 1.22%. Among 162 single root canals, type Ⅰ The detection rate of root canal was 82.72%, the detection rate of type Ⅲ root canal was 1.23%, the detection rate of type Ⅳ root canal was 3.09%, the detection rate of type Ⅴ root canal was 12.96%, Ⅱ, Ⅵ, Ⅶ, Ⅷ root The detection rate of double tube was 0.162%. The detection rate of double root canal was 0.162%. The detection rate of double root canal was 0.166%. Conclusion The occurrence rate of double root canal of the first premolar of human mandible in Shandong Province is relatively high. The morphology of root canal is complex. Attention should be paid to the clinical treatment. Various auxiliary measures can be taken to determine the root canal morphology.
