目的探讨东亚人群中MDM2 SNP309与胃癌风险的关系。方法计算机检索MEDLINE、EMbase和CBM数据库,检索时限均为1990.1.1至2012.12.23,搜集研究东亚人群中MDM2 SNP309与胃癌风险的病例-对照研究。由两位研究者独立进行文献筛选、资料提取和纳入研究的方法学质量评价后,采用RevMan 5.0软件进行Meta分析。结果共纳入5个病例-对照研究,包括1 621例胃癌患者,2 639例对照。Meta分析结果显示:与野生型纯合子(309TT)基因型个体相比,变异纯合子(309GG)基因型个体与胃癌风险增高相关[OR=1.54,95%CI(1.04,2.29),P=0.02],然而杂合子(309TG)基因型个体与胃癌风险增高相关性无统计学意义[OR=1.03,95%CI(0.75,1.42),P=0.006]。在隐形遗传模式中(GG vs.TG/TT)胃癌风险增高且有统计学意义[OR=1.49,95%CI(1.20,1.84),P=0.07],但在显性遗传模式中(GG/TG vs.TT)这种风险增高无统计学意义[OR=1.18,95%CI(0.84,1.65),P=0.001]。结论在东亚人群中,MDM2 309GG基因型个体可能存在较高的胃癌风险。
Objective To investigate the relationship between MDM2 SNP309 and the risk of gastric cancer in East Asian population. Methods The databases of MEDLINE, EMbase and CBM were retrieved by computer. The retrieval time was from 1990.1.1 to 2012.12.23. A case-control study was conducted to investigate the risk of MDM2 SNP309 and gastric cancer in East Asian population. Two researchers independently reviewed the literature, extracted the data, and included the methodological quality of the study. The RevMan 5.0 software was used for meta-analysis. Results A total of 5 case-control studies were included, including 621 gastric cancer patients and 2639 controls. Meta-analysis showed that compared with individuals with homozygous genotype (309TT), homozygous homozygotes (309GG) were associated with an increased risk of gastric cancer (OR = 1.54, 95% CI 1.04,2.29, P = 0.02) However, there was no significant correlation between heterozygote (309TG) genotypes and increased risk of gastric cancer [OR = 1.03, 95% CI (0.75,1.42), P = 0.006]. Gastric cancer risk increased in the stealthy genetic model (GG vs. TG / TT) and was statistically significant (OR = 1.49, 95% CI 1.20, 1.84, P = 0.07) TG vs. TT). No significant difference was found in this risk [OR = 1.18,95% CI (0.84,1.65), P = 0.001]. Conclusion In East Asian population, individuals with MDG2 309GG genotype may have a higher risk of gastric cancer.
