目的:探讨二维及彩色多普勒超声在颈部淋巴结病变检测中的应用价值。方法:用高频超声观察70例患者的92个淋巴结,将淋巴结分为4组:反应性淋巴结、结核性淋巴结、淋巴瘤和转移性淋巴结,将有血供淋巴结的血管模式分为淋巴门型、中央型、边缘型和混合型等4种类型,比较声像图表现在各种病理类型分布中的差异性。结果:恶性肿大淋巴结L/S多<1.8,92.0%的反应性淋巴结和75.6%的淋巴瘤出现淋巴门血流,有75.6%的淋巴瘤出现淋巴结边缘血流;转移性淋巴结有60.0%在淋巴结中央显示血流,57.5%在边缘出现血流;结核性淋巴结的血流分布和恶性淋巴结相似,2个反应性淋巴结、1个结核性淋巴结为无血供。结论:颈淋巴结中央血流和边缘血流主要见于恶性病变,淋巴门血流常意味着良性病变,但是这些血流分布特点在良恶性淋巴结病变中有交叉。
Objective: To investigate the value of two-dimensional and color Doppler ultrasound in the detection of cervical lymph node lesions. Methods: The 92 lymph nodes of 70 patients were observed by high-frequency ultrasound. The lymph nodes were divided into 4 groups: reactive lymph nodes, tuberculous lymph nodes, lymphoma and metastatic lymph nodes. The blood vessel patterns with blood supply lymph nodes were divided into lymphatic vessels , Central type, marginal type and mixed type, etc. 4 kinds of comparison, sonography showed in the distribution of various pathological types of differences. Results: Lymphatic flow was found in lymph nodes of lymph node with lymph node metastasis in 75.6% lymphoma with L / S of more than 1.8% in response to malignant lymph nodes, lymph nodes in 75% of reactive lymph nodes and 75.6% of lymph nodes in lymph nodes, 60.0% in metastatic lymph nodes The central blood flow showed in the lymph nodes, and 57.5% showed blood flow at the edge. The distribution of the nodular lymph nodes was similar to that of the malignant lymph nodes, with two reactive lymph nodes and one tuberculous lymph node without blood supply. Conclusion: The central and peripheral blood flow of cervical lymph nodes are mainly found in malignant lesions. Lymphatic blood flow usually indicates benign lesions. However, the distribution of these blood flow features is cross in benign and malignant lymph nodes.
