应用二氧化碳(CO2)肝动脉造影诊断7例门静脉癌栓,其造影增强声像图表现为:注射CO2微泡造影剂数秒钟后即可见癌栓光团弥漫性光点闪动,似白云翻滚或门脉实体回声内有数条和门静脉管径长轴平行的微气泡光带流动,停止注射后门静脉实体回声内仍有弥漫性非流动性增强光点,持续时间较长。因而可和正常时门静脉管腔呈暗区及肝动脉-门静脉瘘时门静脉内流动性光点相鉴别。我们认为CO2肝动脉超声造影对确诊门静脉癌栓及门静脉癌栓治疗方案的设计有重要作用。
Carbon dioxide (CO2) hepatic artery angiography was used to diagnose 7 cases of portal vein tumor emboli. The contrast-enhanced sonogram showed that: after injection of CO2 microbubble contrast agent for a few seconds, diffuse light spot flashes of cancer bolus can be seen. There are several microbubble bands in the echo of the portal body that are parallel to the long axis of the portal vein. There is still a diffuse non-flowing enhancement spot in the echo of the portal vein after the injection is stopped, and the duration is longer. Therefore, it can be distinguished from the normal light portal vein lumen dark area and hepatic artery-portal fistula when the portal vein fluidity phase differentiated. We believe that CO2 hepatic artery contrast-enhanced ultrasound imaging plays an important role in the design of the treatment of portal vein tumor thrombus and portal vein tumor emboli.
