目的 :探讨幽门螺杆菌 (HP)细胞表达相关抗原阳性 (CagA+)与胃癌的相关性及胃癌发生机制。方法 :选取胃癌 38例、萎缩性胃炎 30例、正常对照 2 4例。应用ELISA法检测血清HPCagA抗体 ,PCR法、组织学方法检测HP感染率 ,同时检测了癌旁组织HP感染率。根据Lauren分型将胃癌组分为肠型 ( 68 4% )和弥散型 ( 31 6% )。结果 :胃癌组CagA+( 50 0 % )明显高于正常对照组 ( 4 1 7% ) (P <0 0 5)。肠型CagA+( 61 5% )显著高于弥散型 ( 2 5 0 % ) (P <0 0 5)。癌旁组织HP感染率 ( 63 2 % )高于癌组织HP感染率( 55 3% )。结论 :萎缩性胃炎及胃癌发生可能与CagA+菌株感染有关。
Objective: To investigate the correlation between gastric cancer-associated antigen (CagA+) expression in Helicobacter pylori (HP) cells and the mechanism of gastric cancer. Methods: 38 cases of gastric cancer, 30 cases of atrophic gastritis and 24 cases of normal controls were selected. The serum HPCAgA antibody was detected by ELISA. The infection rate of HP was detected by PCR and histological methods. The rate of HP infection in the adjacent tissues was also detected. According to the Lauren classification, the gastric cancer components were intestinal (68.4%) and diffuse (31.6%). Results: The CagA+ (50 %) in gastric cancer group was significantly higher than that in normal control group (41.7%) (P <0 05). The intestinal CagA+ (61 5%) was significantly higher than that of the diffuse type (2.50%) (P < 0.05). The incidence of HP infection in cancer tissues (63.4%) was higher than that in cancer tissues (55.4%). Conclusion: Atrophic gastritis and gastric cancer may be related to the infection of the CagA+ strain.
