目的探讨血清胆红素水平对新生儿心肌的影响。方法选取2015年7月-2016年6月在该院就诊的122例新生儿黄疸患儿作为研究对象,按日龄分为≤14 d、>14 d两组,每组按血间接胆红素水平高低分为≤256.5μmol/L、256.5~307.8μmol/L、307.8~342μmol/L、≥342μmol/L 4小组,分别将数值进行横向及竖向组间比较分析。结果 (1)≤14 d组,随胆红素水平增高,血间接胆红素水平在307.8~342μmol/L时超敏肌钙蛋白T(CTnI)浓度出现下降,与其他组比较差异有统计学意义(P<0.05);血间接胆红素水平在256.5~307.8μmol/L时肌酸激酶MB(CK-MB)浓度也出现下降,差异有统计学意义(P<0.05);其他3小组CK-MB及CTn I变化无统计学意义(P>0.05)。(2)>14d组,不同胆红素水平CK-MB及CTnI变化无统计学意义(P>0.05)。(3)两组竖向比较同等胆红素水平下,>14 d组较≤14 d组CK-MB及CTn I水平明显下降,差异有统计学意义(P<0.05)。结论新生儿血清胆红素水平与心肌标志物的变化无明显相关性;血清胆红素在一定范围内可能对心肌细胞有保护作用;更高水平的胆红素是否对心肌造成损害有待进一步研究。
Objective To investigate the effect of serum bilirubin on neonatal myocardium. Methods A total of 122 neonates with jaundice from July 2015 to June 2016 were enrolled in this study. Patients were divided into two groups according to their age of ≤14 days and> 14 days, The levels were divided into groups of ≤256.5μmol / L, 256.5 ~ 307.8μmol / L, 307.8 ~ 342μmol / L, ≥342μmol / L.4 The values were compared between horizontal and vertical groups respectively. Results (1) In 14d group, with the increase of bilirubin level, the concentration of CTnI in serum was decreased from 307.8 to 342μmol / L, with statistical significance compared with other groups (P <0.05). The concentration of creatine kinase MB (CK-MB) also decreased when the blood indirect bilirubin level was between 256.5 and 307.8μmol / L, the difference was statistically significant (P <0.05) -MB and CTn I change was not statistically significant (P> 0.05). (2)> 14d group, CK-MB and CTnI of different bilirubin levels had no significant difference (P> 0.05). (3) The levels of CK-MB and CTn I in the group of> 14 d were significantly lower than those in group of ≤ 14 d under the same level of bilirubin in both groups (P <0.05). Conclusions The serum bilirubin levels in newborns have no significant correlation with changes of myocardial markers. Serum bilirubin may have a protective effect on cardiomyocytes in a certain range. Whether higher levels of bilirubin cause damage to myocardium remains to be further studied .
